  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        TVCMHP/MATCH exists to increase access to mental health services for low income children. The Treasure Valley of Southwest Idaho has 25 fewer child psychiatrists today than in 1995 while the population has increase 50% since 1990. Low income children wait 8 to 12 weeks to see a child psychiatrist.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Ongoing training, continuing education and the development of resources for primary care physicians in the treatment of mental health disorders.
        Objective 1: Conduct 12, 60-minute continuing education sessions to primary care physicians on different aspects of mental health care and treatment modalities.
        Objective 2: Host/sponsor a revenue generating conference for health care providers on mental and behavioral health issues and care.
        Objective 3: Create and maintain an internet-based resource and referral site for primary care providers. Produce and distribute resource manual.
         
        Goal 2: Development and implementation of a cross-service system for primary care physicians providing mental health care and treatment to children.
        Objective 1: Develop and secure all necessary MOUs and MOAs for services to be provided as well as partnership agreements and responsibilities. Recruit two additional primary care physicians to the program.
        Objective 2: Provide treatment care plans with a psychiatric consult for 104 patients to participating primary care physicians. Evaluate the service for improvement and modify services accordingly.
        Objective 3: Continue facilitation of Board/collaborative partners engagement and involvement.
         
      3. Activities/Methodology undertaken to meet project goals:

        The TVCMHP project will train primary care physicians in best-practice research-based protocol and evaluation/assessment tools. Participating physicians will administer screening assessments and then refer to Saint Alphonsus for appropriate care which may include referral to MATCH. MATCH will provide quick access to the physician as well as the patient and family to a clinical psychologist and child psychiatrist. A treatment plan will be crafted and provided to the participating physician and patient family for management.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.

        1. Related to Objective 16.23. Increase the proportion of Territories and States that have service systems for Children with Special Health Care Needs to 100 percent.
        2. Related to Objective 16.23: Increase the proportion of States and jurisdictions that have service systems for children with or at risk for chronic and disabling conditions as required by Public Law 101-239.
        3. Related to Objective 23.11 (Developmental) Increase the proportion of State and local public health agencies that meet national performance standards for essential public health services.

      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

        St. Luke’s RMC Coordination and Implementation of Primary Care Physician Training & Continuing Education & Project Funding Saint Alphonsus RMC; Screening, Triage, Referral and Transcription & Project Funding Children’s Home Society/WSCC; Care Coordination, Fiscal Agent of HTPC funding & Project Funding Idaho DHW; Evaluation Services & Project Funding & Project Funding Idaho Federation of Families for Children’s Mental Health; Office Space, Staff Support & Project Funding

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        Evaluation of process and outcomes will be conducted by the Idaho Department of Health and Welfare. Simple measures will include patient quantities, patient demographics, patient satisfaction, physician satisfaction, process improvement recommendations, and cost analysis.

    2. Continuing Grants ONLY

      1. Experience to Date :

        The primary accomplishment to date is the incorporation of a nonprofit organization with consensus on and mutual commitment to a system of integrated service delivery among typically contentious organizations. Eighteen (18) primary care physicians from the area have volunteered to participate in the project. MOUs have been developed and are in process. An orientation for participants is scheduled for November 10th. Baseline data has not been collected to determine whether or not a specific number of additional low income patients will be provided for. At this point it has been determined that the number of patients utilizing MATCH services will be tracked and increases will be monitored for MATCH participants only. Two (2) one-hour training/continuing education sessions have been conducted and will continue at St. Luke’s. Total attendance for both sessions equaled 19. Videotapes of the sessions are available for those unable to attend. In addition, a full-day conference will be conducted May 6, 2006 with an anticipated attendance of 200+ medical professionals.

  6. Annotation:

    The TVCMH will increase access to mental health services for high risk, low income children by enhancing the capacity of primary care physicians to serve these children. Strategies include: regular training for primary care physicians; creation of county-specific guide to resources available for pedicatric mental health patients and their families; professional support services including triage, psychiatric consultation and evaluation, and case management for participating primary care physicians.
