  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        The lack of specific information on Calif. CSHCN population severely hampers access to policy/program information. The State provides policy direction and oversight and counties determine eligibilty. This division has resulted in the lack of Statewided specific information on the CCS program.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: The CMS Branch's mission is to assure the health of California children. The SSDI grant goal is to provide accurate, child-specific, accessible information on California's CSHCN through CMS Net/E47.
        Objective 1: (1) Ensure the database is Statewide by completing the transition of counties using stand-alone systems to CMS Net. (2) Perform ongoing system maintenance of the CMS Net system.
        Objective 2: (3) Develop and implement the Provider Inquiry Project to allow providers to verify coverage and service authorizations of CSHCN in CMS Net. (4) Develop a claims payment management system and a post-payment monitoring system within the Business Objects software.
        Objective 3: (5) Linking data on CCS children from CMS Net with other State databases, especially hospital discharge data and data from the Newborn Hearing Screening Program (NHSP).
         
      3. Activities/Methodology undertaken to meet project goals:

        The CCS Program provides case management and assures and authorized direct health care services for California residents up to age 21 who meet the medical and financial eligibility requirements of the program. Medical eligibility for CCS includes almost all serious or chronic medical conditions that can be stabilized, improved, or cured such as birth defects, genetic diseases, physically handicapping conditions, chronic illnesses, and severe injuries. The CMS Net system includes the functions of child registration; eligibility determination and annual re-determination; tracking case management events; generation of letters requesting family follow up and medical information from providers; authorization of services, and narrative information documenting and supporting coordination of the child's care. Through this SSDI project, CMS Net is being expanded to include all 58 California counties and the entire CCS population. CMS Net is also being linked to other State data systems such as thos for Medi-Cal (California's Medicaid Program) and the State's Fiscal Intermediaries. This database provides a wealth of information on CSHCN in Calif. that can be helpful in analyzing the CCS population and those children with CSHCN who were found to be ineligible for the program. Additionally, extensive data on services for CCS children exists from information from CCS provider claims processed the the fiscal intermediary.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.


      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

        State and county (58) collaboration on data systems is ongoing and integral to the expansion of CMS Net. Linkage of CMS Net to other State systems is currently being carried out in conjunction with Medi-Cal and with the State Fiscal Intermediaries.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        For each activity undertaken under the SSDI project, project work-plans are established and monitored on a weekly basis. For the county conversion project, progress reports are prepared on a monthly basis. Meetings are held weekly and monthly to discuss project issues. These activities provide a structured mechanism for determining the status of the county conversions.

    2. Continuing Grants ONLY

      1. Experience to Date :

        Significant progress has been made toward the specific objectives targeted for this year. As of April 2004, 55 of California's 58 counties have fully converted to CMS Net. Current plans are to have all of California's 58 counties fully converted by September 2006. We sucessfully implemented the CCS provider enrollment and insurance components of CMS Net/E47 in March 2004. The State Fiscal Intermediaries have completely revised their systems to receive electronic authorizations from CMS Net and began to accept such authorizations on July 2004. CMS continuously uses Business Objects as our report writing tool. As of July 2005, we have increased the user population to 55 counties participating with a total of 300 users using this tool for CCS needs and 58 counties participating with a total of 307 users (State and county combined) using this tool for CHDP and CHDP Gateway needs.
