  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        The project provides an interdisciplinary assessment for the children and families of York County, Maine experiencing behavioral and mental health disorders via the York County Learning & Behavior Assessment Clinic, providing for a more coordinated system of care and improving access to services.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Establish formal links with DHHS to provide team assessment, medical review, diagnosis, and treatment planning for high risk populations.
        Objective 1: Secure a formal contract with DHHS for assessment services for children in the foster care system in Maine.
        Objective 2: Particpate in the York County Prevention Collaborative in its efforts to develop centralized community goals for prevention for children in York County.
        Objective 3: Establish formal links with the Juvenile Justice System to provide team assessment, medical review, diagnosis and treatment planning for children involved with the Juvenile Justice System.
         
      3. Activities/Methodology undertaken to meet project goals:

        Continue to meet with DHHS and Juvenile Justice representatives to finalize contract terms, with a particular focus on demonstrating the cost-effectiveness of interdisciplinary assessment services as an alternative to costly inpatient stays. Collaborate to determine how state funds could best be leveraged for maximun benefit.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.

        1. Related to Objective 16.23. Increase the proportion of Territories and States that have service systems for Children with Special Health Care Needs to 100 percent.
        2. Related to Objective 16.23: Increase the proportion of States and jurisdictions that have service systems for children with or at risk for chronic and disabling conditions as required by Public Law 101-239.
        3. Related to Objective 23.11 (Developmental) Increase the proportion of State and local public health agencies that meet national performance standards for essential public health services.

      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        An evaluation plan has been developed and implemented which includes patient satisfaction surveys, follow-up phone calls, a quality of life questionnaire, and a comprehensive patient data base. We are currently in the process of collecting this information to determine to what extent we are meeting the needs of our clients and how we could improve the quality of services deliverd.

      1. Experience to Date :

        The Clinic has been in operation since October 2003 and is currently serving a caseload of 275 new or follow-up patients per year. Our focus these past few years has been on developing an effective infrastructure to support Clinic operations (including an automated patient data base), marketing services to the community, developing an evaluation plan and ensuring that the activities of the Clinic are coordinated with existing vehicles and groups in York County interested in improving access to mental health services for children, such as the United Way of York County and state governmental representatives. We are focusing future efforts on developing sustainable sources of funding through formal contracts (e.g. children in the foster care or juvenile justice system) and enhanced provider billing mechanisms.
