  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        Many different agencies and divisions throughout state government provide a broad range of support services. Since many service recipients are enrolled in multiple programs, the efficiency and effectiveness of those programs can be greatly improved by linking presently independent information.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Support the Title V Block Grant Core Health System Capacity Indicator #9(A)
        Objective 1: Provide timely data for the MCH Block Grant as needed.
        Objective 2: Provide analytic capability in support of MCH Block Grant activities.
         
        Goal 2: Provide ongoing needs assessment, performance, and outcome measures.
        Objective 1: Provide linkage data necessary to compute quarterly performance measures for newborn genetic screening and newborn hearing screening.
        Objective 2: Provide quarterly linkages to hospital discharge data to ensure quality of hospital and birth certificate data.
        Objective 3: Provide quarterly linkage between birth certificates and death certificates to ensure completeness of assessment of infant and maternal mortality.
         
        Goal 3: Use the linked MCH-related data systems to identify trends, risk factors, utilization of prenatal care, and health status outcomes in maternal and child health.
        Objective 1: Monitor pregnancy intent, birth rates, close birth spacing, high birth order, etc. in the Medicaid population to assess family planning activities, abstinence education.
         
      3. Activities/Methodology undertaken to meet project goals:

        In conjunction with another HRSA/MCHB DUE Cooperative Agreement, SAS is used to link Birth Certificates with nine other MCH-related (Maternal and Child Health) databases: (1) Death Certificates, (2) Medicaid, (3) WIC, (4) Newborn Metabolic Screening, (5) Hospital Discharge Data System, (6) Birth Defects Registry, (7) Pregnancy Risk Assessment Monitoring System (PRAMS), (8)CSHCN (Children with Special Health Care Needs), and (9) Newborn Hearing Screening. Youth Risk Behavior Surveys are being done in counties and school districts throughout the state.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.


      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

        The CHS works closely with several MCH-related divisions within the Arkansas DHHS. Consumer, the Advisory Board provides advocacy and provider policy perspectives for Perinatal Health Services, a pre-existing, statutorily mandated board. Additional input is obtained from Arkansas Advocates for Children and Families and the Arkansas School Readiness Indicators Initiative. Relations have been strengthened with the Arkansas Primary Care Cooperative Agreement Office and the CISS program.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        The Arkansas State Systems Development Initiative is a capacity building endeavor, designed to strengthen the MCH data and information infrastructure of Arkansas. As such, process evaluation is used to assess the quality, completeness and timeliness of the linked databases, and responsiveness to MCH Block Grant data requirements.

    2. Continuing Grants ONLY

      1. Experience to Date :

        For the purposes of this project, a January 1, 1997 birth date was selected as the earliest date for most of the file linkages. To date, linkage of birth certificates to death certificates, hospital discharges, PRAMS, birth defects, newborn metabolic screening, newborn hearing screening and WIC are current. Requests for access to Medicaid and CSHCN files are being prepared. The Arkansas Legislature directed Arkansas Department of Health to merge with the Department of Human Services, effective July 1, 2005. The Governor specifically mentioned the ability of the Center for Health Statistics to provide policy analysis capability to DHS when he addressed the Legislature on the merger. As a result, we are anticipating greater access to the Medicaid data files.

    The State Systems Development Initiative are devoted to linking MCH-related data systems, they have been fully integrated, resulting in significantly improved efficiency. A great deal of progress has been made in linking files. The linked data systems will be used to identify trends and health disparities in maternal and child health at the state and regional levels at the State of Arkansas
