  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        The Emotional Health Screening Program, screens Fond du Lac county students, for signs and symptoms of mental health problems and suicide tendencies utilizing Columbia University's TeenScreen Program

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Improve the overall emotional health of students throughout Fond du Lac County by effectively screening them for mental health problems and suicide risks.
        Objective 1: By May 2006, all students enrolled in Fond du lac School District will, by the age of 18, have access to emotional health screening and subsequent referral and treatment
        Objective 2: By May 2007, all students enrolled in private school system within the City of Fond du Lac will, upon referral, have had access to emotional health screening and subsequent referral and treatment
        Objective 3: By February 2009, all students ennrolled in public and private school systems with Fond du Lac County will, upon referral or screening site,by the age of 18, have access to emotional health screening and sebsequent referral and need treatment
         
      3. Activities/Methodology undertaken to meet project goals:

        (1) Continue and refine TeenScreen Programming ( Year 1 sites) at Fond du Lac High School, Horace Mann High School and St. Mary’s Spring High School (9th grade) (2) Continue and refine TeenScreen Programming ( Year 2 sites) Oakfield, Campbellsport and St. Lawrence Seminary (9th grade) (3) Agnesian HealthCare and Aurora Health Center pediatric and family practice TeenSreen sites will be enhanced, refined and continued. (4) Identify and establish a TeenScreening programming at a pedicatrican clinic in Ripon to serve youth in western FDL county (5) Continue and refine TeenScreen site at Alternative High Schools at Fond du Lac School District and North Fond du Lac School District (6) Cotinue and refineThe Middle School TeenScreen site at Fond du Lac School District (7) Establish one additional TeenScreen pilot site at a public or private school in Fond du Lac county, to screen 9th grade and other referred students in that area. (8) Explore the possibility of establishing an additional TeenScreen site at the Fond du Lac County Courts – Juvenile Intake Program. (9) Provide TeenScreen training to school administrators, Marian College students, mental health providers, pediatricians and family physicans (10) Recruit and maintain mental health providers volunteering for TeenScreen Program

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.

        1. Related to Objective 16.23. Increase the proportion of Territories and States that have service systems for Children with Special Health Care Needs to 100 percent.
        2. Related to Objective 16.23: Increase the proportion of States and jurisdictions that have service systems for children with or at risk for chronic and disabling conditions as required by Public Law 101-239.
        3. Related to Objective 23.11 (Developmental) Increase the proportion of State and local public health agencies that meet national performance standards for essential public health services.

      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        The evaluation plan has strategies for closely monitoring the activities, and assesses and measures both the short and long-term impact of those activities. The Project Director, with assistance from Columbia University’s TeenScreen program specialists, TeenScreen Co-Coordinators area health care professionals, and the Advisory Committee, will collect both formative and summative data at regular intervals through the five-year project.

    2. Continuing Grants ONLY

      1. Experience to Date :

        The emotional health screening program was provided to 9th grade students enrolled at Fond du Lac High, St. Mary Springs High School, Horace Mann High School and Alternative High School for mental illness (including ATOD use) and risk for suicide, and ensure subsequent referral for treatment. The program utilizes the scientifically proven TeenScreen Program developed through Columbia University. Since the inception of the program, 1098 students from Fond du Lac County have been screened. Approximately 295 students have been referred who have been identified for being at risk of suicide or suffering from mental health problems and received appropriate for treatment. During the last year, Eight hundred eighty-three Fond du Lac County youth were offered the opportunity to participate in the TeenScreen, 518 students completed the screen and 95 students were referred for services. During year two, tremendous progress has been made in implementing the Emotional Health Screening Program – TeenScreen program at Aurora Health Center and Agnesian HealthCare. For those youth or parents who do not feel comfortable participating in the TeenScreen at school, the primary care setting offers another opportunity to participate in screening. The Columbia Teen Screen Program is administered to patients in 6th grade and above during their scheduled physical or on an as-needed basis for those with immediate mental health concerns.

  6. Annotation:

    Fond du Lac County High School students report having seriously thought about committing suicide. The Emotional Health Screening Program screens youth for risks factors assoicated with depression, anxiety, substance abuse and suicide ideation. The goal of the program is that all Fond du Lac youth would be offered to participate in the screen before 18 years of age. The primary objective is to help youth and parents through early identification. This school -community based program will develop screening sites in schools and primary care settings.
