  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        Currently three-fourths of the births at Parkland are to Hispanic mothers, the majority of them are low-income. Many of these mothers are considered at-risk because the are first time mothers, teen mothers, mothers of a special needs child or deemed to be physically or emotionally vulnerable.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Reduce disparities in health care by promoting sound child-rearing practices, providing Spanish-language, culturally sensitve health education for vulnerable mothers.
        Objective 1: By 2/28/08, increase to at least 45% the number of enrolled infants who are currently breastfeeding or were breastfed until the age of one year.
        Objective 2: By 2/28/08, increase to at least 70% the number of enrolled patients who have fundi9ng for medical are through Medicaid, the Children's Health Insurance Program (CHIP) or private insurance.
        Objective 3: By 2/28/08, increase to at least 40% the number of parents of enrolled infants who score at least 70% correct on measures of knowledge of infant development after nine to twelve months of program participation.
         
      3. Activities/Methodology undertaken to meet project goals:

        Employ trained bilingual paraprofessionals to work with vulnerable mothers and their children.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.

        1. Related to Objective 16.23. Increase the proportion of Territories and States that have service systems for Children with Special Health Care Needs to 100 percent.
        2. Related to Objective 16.23: Increase the proportion of States and jurisdictions that have service systems for children with or at risk for chronic and disabling conditions as required by Public Law 101-239.
        3. Related to Objective 23.11 (Developmental) Increase the proportion of State and local public health agencies that meet national performance standards for essential public health services.

      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

        Babies First! is administered through the Community Oriented Primary Care clinics. Site Administrators, pediatricians, social workers and nurses interact with the child advocates daily. Community Advisory Boards are directed by the Community Coordinator.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        At each contact with the family, the child advocate will determine if the infant is being breastfed and if there is funding for health care. Between nine and twelve months of age, the infants' parents will be asked to complete a questionnaire regarding knowledge of child development.

    2. Continuing Grants ONLY

      1. Experience to Date :

        Objective A: By 2/28/06, 40% (120) of 300 enrolled infants will be currently breastfed. To date 101 of the 174 eligible infants are breastfed. This is fifty-eight percent, which exceeds both the budget year and the project period goals. Objective B: By 2/28/06, 65% (195) of 300 enrolled patients will have funding for medical care. One hundred seventy-five of the 180 enrolled infants have funding for medical care. The vast majority of them are on Medicaid. Thus ninety-seven percent of the enrollees are funded, exceeding both the budget year and project period goals. Budget Year3 Objective C: By 2/28/06,30% (90) of 300 parents of enrolled infants will score at least 70% correct on measures of knowledge of infant development after nine to twelve months of program participation. This objective was not achieved due to the high turnover in the child advocate positions. This is also the reason that the targeted number of enrollees for each of the objectives has not been met. Some parents have completed the infant development assessment but the scores are not available.

    Babies First! will provide culturally sensitive health education to vulnerable mothers whose babies are newborn through age two years. Strategies include using trained, bilingual paraprofessionals supervised by the pediatric team to provide ongoing health education, home visits, case management, and access to community resources.
