Background: A goal of the grant programs supporting services for children and youth with special health care needs is to achieve critical systems outcomes set out in the Healthy People 2010 Goals that were developed to eliminate barriers to community inclusion for people with chronic health conditions and disabilities. A community based system of services is comprised of six outcomes:
Programs collect the data for those MCHB grantees involved in State-wide processes for implementation for such activities like participating in state-level task force, working with other State agencies, families and community partners, providing technical assistance or consultation, and providing financial resources to communities for systems development, among others. These data are collected on the CSHCN data form.
