This guide encourages adult women living in rural areas to increase current levels of physical activity, healthy eating, and communication with their health care providers to set goals for behavioral changes. It features a self-questionnaire to assist in determining current levels of physical activity and healthy eating, a section to list questions health care providers, a body mass index table, and tools for reaching healthy goals. The guide includes a food and activity journal, a form to set goals, and referrals for additional information. As a companion to the guide, a set of ten Tip Sheets that expand on the topics and provide practical information, ideas, and activities to help women adopt healthy behaviors to reach their goals, are available online at:
