Abstract:	This report presents findings from an effort to identify, engage, and retain individuals in care, who demonstrate a very high level of unmet needs. It addresses issues of HIV service delivery for diverse minority populations, those who are homeless, geographically isolated, chemically dependent, or those with language barriers, or who are attempting to navigate new managed care systems.
