Private Practice Option Application
This PPO application initiates a request for a NHSC clinician to open their own private practice or one who is interested in working at an existing private practice. Just by submitting a PPO application does not allow a physician to be approved for the private practice option. Approvals of all PPO applications will be sent in writing from the In-Service Support Branch. In addition to filling out the PPO application, additional documentation will be required. See documentation requirements for private practice options.

Six Month Verification Forms
Your forms are sent via e-mail on or around the last day of your six month period. If your obligation is ending, your Six Month Verification Form will arrive approximately two weeks prior to your obligation end date. Please contact the National Health Service Corps – Recruitment Training & Support Center (NHSC-RTSC) at RTSC@hrsa.gov to update your e-mail address. If you need a 6 Month Service Verification form or want to check the status of your Six Month Verification, please contact the NHSC-RTSC at this email address.
