Collaborative Aim:

by achieving optimal health outcomes and eliminating adverse drug events through increased clinical pharmacy services for the patients we serve.”
To ensure that patient care delivered by HRSA grantees
and other health care providers serving poor, uninsured
and underserved people becomes the safest and best
in the nation, HRSA is studying leading practices
in patient safety, clinical pharmacy services and
health outcomes.
The goal is to replicate those leading practices
across HRSA-funded health care providers and their
partners using a collaborative model based on the
methodology of the Institute for Health Care Improvement.
HRSA intends to pull together teams of health care
providers from communities across the nation to learn,
replicate, test, and adopt specific practices in patient
safety and clinical pharmacy services to improve health
outcomes and reduce adverse events.
| September 2008-December 2009 |
- Hold three additional Learning Sessions: December 3-4 2008, May 6-7 2009, Fall 2009.
- Enrolled teams generate rapid improvements using the leading practices shared during Learning Sessions.

- Held first Learning Session, in which enrolled teams, with help from expert faculty, will receive peer-to-peer technical assistance and opportunities for collaboration.

- July 1, 2008: Teams of community-based health care providers enrolled in the Collaborative.

| Spring 2008 |
- Established ambitious quantitative and qualitative aims to guide improvement in clinical pharmacy services, patient safety and health outcomes.
- Convened a faculty of health care providers (identified during the study phase) and experts from world-class quality improvement organizations.

- Studied high performing health care organizations and their leading practices for achieving exceptional health outcomes and reducing adverse events.
