- Download and save the Form 80 PDF file shown at the link on the right side of this page. The form is labeled Form 80 (for efilers). This “fillable” form file can be used to enter all data.
- Complete the form data entry process off-line.
- Complete the process by filing the Form 80 electronically with the Commission.
