PHIN Certification certifies the ability of an application (or multiple applications, components, or systems) to perform specific functions in compliance with the PHIN Requirements and Certification Criteria. The criteria for certification, the PHIN Certification details, and a general description of the PHIN Certification process are available in PHIN Certification Criteria and Process v1.0 document.

How to Prepare for PHIN Certification

  • Review the details for the individual PHIN Certification (“Available Certifications” tab above)
    • Identify the applicable PHIN Requirements and PHIN Certification Criteria


    • Identify the applicable guides and specification documents

  • Use the applicable guides and specification documents to determine the specific detailed requirements for implementation


  • Use the available PHIN tools to help prepare for PHIN Certification (“Tools” tab above)
    • Use PHIN Certification Security and Availability Assessment Tool to self-assess compliance with the security and availability certification criteria (PHIN Requirement #5)


    • Use the specific PHIN Certification evaluation readiness checklist

  • Review the specific PHIN Certification Evaluation documents detailing the specific evaluation steps to ensure systems have the necessary functionality (“Available Certifications” tab above)

  • Request assistance from PHIN Support Team or other CDC support resources as needed.

 



How to Apply for PHIN Certification

Complete the PHIN Certification Review Request Form Word Document and submit it to the PHIN Certification Group (phincertification@cdc.gov). Indicate on the form which PHIN Certification(s) your jurisdiction is applying for applying for and provide the contact information requested.

The PHIN Certification Group will contact the primary contact listed on the form to explain the process, answer questions about the process and schedule the necessary PHIN Certification evaluation session(s).

 



Typical PHIN Certification Evaluation Process and Time Estimates

    Partner requests PHIN Certification evaluation
  • Complete and submit PHIN Certification Evaluation Request Form

Minimal
    PHIN Certification Group (PCG) contacts partner
  • Schedule time for an introductory call with partner to review evaluation process and ensure partner is ready for the certification evaluation

  • Establish tentative schedule for evaluation session(s)

  • Ensure partner is ready for the certification evaluation


  • Finalize the evaluation session times and dates based on partner’s and PCG’s availability

    PCG supplies partner test data
  • PCG sends partner the test data necessary to complete the evaluation steps (2+ business days prior to the evaluation session)

    Partner completed the evaluation steps
  • Complete, sign and submit the PHIN Security and Availability Self-Attestation Form to the PCG


  • One or more sessions to complete the evaluation steps to demonstrate compliance with the criteria


Typically one or two sessions, each session approx. 1-3 hours

All session(s) typically completed within a one week period
    PCG provides partner with PHIN Certification Evaluation Report
  • PCG send the partner the report within 2-3 business days after completion of the evaluation sessions


  • Report will indicate whether the partner passed or failed the certification evaluation


  • For failed evaluations the deficiencies or non-compliance items will be listed


None
    Re-evaluation Period
  • Partner has 30 days from the date of the report to correct the deficiencies or non-compliance items and be re-evaluated on the failed evaluation steps


  • One or more sessions to complete the failed evaluation steps. Session schedule determined by PCG and partner.

    PHIN Certification Awarded
  • When certification evaluation is passed the partner will be awarded the PHIN Certification
