Quinidine-doxycycline should be given because
the clinical presentation suggests severe malaria due to Plasmodium
falciparum, and the patient should be treated accordingly.
The report of P. vivax on the blood smear may reflect
a mixed infection or misdiagnosis. Severe P. falciparum must
be treated aggressively.
