NOTICIAS DEL NIEHS | Los pesticidas y los niños
Los estudios en animales indican que la exposición prenatal y en los primeros días tras el nacimiento a los contaminantes afecta de forma negativa a la salud respiratoria y al desarrollo físico y cognitivo. Investigadores de ocho centros de salud infantil de Estados Unidos, patrocinados en conjunto por el NIEHS y la Agencia de Protección Medioambiental de EE. UU., llevan a cabo estudios en edades tempranas para determinar los efectos de la exposición ambiental sobre la salud humana. Este artículo (p. A664) trata sobre el trabajo que se está realizando en cuatro de estos centros, que evalúan y combaten los efectos de los pesticidas, entre otros contaminantes.
ENFOQUE | Efectos relacionados con el sexo de los EDC
Se sabe que la exposición a altas dosis de sustancias químicas perturbadoras del sistema endocrino (EDC) pueden producir efectos dañinos para la salud; pero ¿cuáles son los efectos adversos, si los hay, por la exposición a dosis bajas, especialmente durante el desarrollo fetal? Aunque algunos expertos no se arriesgan a opinar, en espera de evidencias más concluyentes, existen dos puntos de vista opuestos sobre la influencia de la exposición a dosis bajas de EDC. En este artículo (p. A670) se examina el debate existente entre los que afirman que no hay motivos de alarma y los que citan evidencias de malformaciones en los organos sexuales humanos, en el desarrollo y el comportamiento asociado al género, las capacidades reproductivas y la relación entre sexos.
ESFERAS DE INFLUENCIA | Borrador europeo de referencia sobre productos farmacéuticos
Los productos farmacéuticos, aun siendo de gran ayuda para luchar contra las enfermedades, pueden suponer un peligro para la salud ambiental y humana si acaban en el desagüe. Para ayudar a controlar la dispersión de estos compuestos químicos en el medio ambiente, la Agencia Europea de Medicamentos (EMEA) ha publicado un borrador de referencia sobre un procedimiento de evaluación de riesgos medioambientales que deberá incluirse en las solicitudes de las empresas farmacéuticas para ser autorizadas a sacar al mercado nuevos fármacos. Este artículo (p. A678) describe los detalles de esta referencia de la EMEA, la compara con un protocolo similar utilizado en Estados Unidos y explica sus posibles ventajas y limitaciones.
NOVEDADES | Alternativas a las soldaduras de plomo-estaño
Las soldaduras de plomo-estaño son un elemento básico, fiable y muy extendido, en la manufacturación de productos electrónicos; pero también son bien conocidas como un peligro medioambiental. En estos momentos, tras la prohibición de la Unión Europea de todo uso del plomo en los equipos electrónicos que entrará en vigor el 1 de julio de 2006, los investigadores buscan materiales menos peligrosos, aunque igual de confiables, para las soldaduras. En este artículo (p. A682) se analizan los nuevos estudios científicos sobre posibles alternativas a la aleación de plomo-estaño que harán que los dispositivos electrónicos sean de utilidad tanto durante su vida de servicio como posteriormente.

IDENTIFICACIÓN DE RIESGOS | Puntos débiles de la hormesis para la toma de decisiones sobre la salud pública
La hormesis (estimulación con dosis bajas e inhibición con dosis altas) se suele utilizar para sustentar la idea de que, si bien la exposición a altos niveles de productos químicos podría ser perjudicial para la salud humana, la exposición a niveles bajos podría ser beneficiosa. Thayer et al. (p. 1271) han evaluado la hipótesis de la hormesis y las consecuencias adversas potenciales que supondría tener en cuenta los efectos beneficiosos de las dosis bajas en las tomas de decisiones sobre salud pública. Los autores argumentan que asumir que la hormesis se puede adaptar de forma general es una enorme simplificación de procesos biológicos complejos.

ENDOCRINOLOGÍA | Actividad modulada por los RE de los PCB
El impacto de los bifenilos policlorados (PCB) sobre el señalamiento estrogénico no se comprende aún en su totalidad. Plísková et al. (p. 1277) han utilizado el bioensayo ER-CALUX para determinar las actividades estrogénicas/antiestrogénicas de los congéneres del PCB más habituales y de las mezclas aisladas de muestras de suero de varones humanos recogidas en residentes de un área con una intensa contaminación medioambiental y han comparado los resultados con los de regiones de referencia vecinas en Eslovaquia oriental. Los PCB poco clorados resultaron ser estrogénicos, mientras que los congéneres del PCB altamente clorados más habituales y los principales metabolitos del PCB se comportan como antiestrógenos. Los PCB coplanares no tienen efecto directo en la activación del receptor de estrógenos (ER). Los datos sugieren que podría ser importante investigar los efectos directos de los PCB en los niveles de la hormona esteroidea en los sujetos muy expuestos.
ENDOCRINOLOGÍA | Los efectos del humo del tabaco en las hormonas
El humo del tabaco contiene compuestos que se piensa que pueden afectar a la actividad hormonal. Windham et al. (p. 1285) han medido los niveles en la orina de la hormona folículo estimulante de la pituitaria (HFE) en un subconjunto de ciclos menstruales, seleccionados según el nivel de tabaquismo, tomando como elemento de principal interés el tiempo de transición entre dos ciclos. Los niveles medios diarios de HFE en la orina alrededor de la transición del ciclo se incrementaban al menos un 30-35 % con un tabaquismo moderado. El humo del tabaco puede alterar la función endocrina, probablemente debido a su efecto en los ovarios, lo que a su vez afecta a la liberación de las hormonas pituitarias. Es probable que esta perturbación endocrina contribuya a la asociación entre el tabaco y los efectos reproductivos adversos, como la disfunción menstrual, la infertilidad o una menopausia más temprana.
DESARROLLO | La exposición a los clorpirifos durante el desarrollo
La exposición a los clorpirifos durante el desarrollo altera la señalización celular en el cerebro y en los tejidos periféricos. Slotkin et al. (p. 1291) administraron 1 mg/kg/día de clorpirifos a ratas en los días 1 al 4 después del nacimiento, una dosis por debajo del umbral de toxicidad sistémica. Cuando se realizaron pruebas en los adultos, los animales que habían sido expuestos a clorpirifos mostraban elevaciones en los niveles plasmáticos del colesterol y los triglicéridos, pero no alteraciones de los ácidos grasos libres no esterificados ni del glicerol. Este efecto se limitaba a los machos. Los resultados también indican que la exposición neonatal subtóxica a los clorpirifos, aparte de los efectos sobre la viabilidad o el crecimiento y dentro de los parámetros de las exposiciones humanas fetales y neonatales, produce una alteración en el perfil lipídico plasmático y la insulina que se asocia a los factores de riesgo para la aterosclerosis y la diabetes mellitus de tipo 2.
SALUD DE LA POBLACIÓN | Desequilibro en la proporción de sexos en humanos
Los miembros de la Comunidad Nacional Aamjiwnaang de Canadá han expresado su preocupación al comprobar que parece haber menos niños varones en su comunidad en los últimos años. Mackenzie et al. (p. 1295) han evaluado la proporción de nacimientos de varones en el periodo 1984-2003 como parte de un proyecto de investigación participativo basado en la comunidad. La tendencia en la proporción de nacimientos de varones vivos de la Nación Aamjiwnaang ha ido disminuyendo de forma continua desde principios de los años 90 hasta el 2003. Anteriormente parecía existir una distribución por sexos equilibrada. Se está llevando a cabo actualmente una inspección sanitaria en la comunidad para recabar información sobre la salud de la Nación Aamjiwnaang y para investigar los factores que pueden estar contribuyendo al descenso de la proporción de nacimientos de varones.

ENDOCRINOLOGÍA | Sensibilidad a la exposición a aguas residuales depende de las etapas de la vida de las carpas
Las inspecciones de los ríos británicos indican que existe una perturbación sexual en las poblaciones de carpas salvajes (Rutilus rutilus) que viven río abajo de las plantas de tratamiento de aguas residuales. Liney et al. (p. 1299) han investigado las respuestas feminizantes a dos plantas de tratamiento estrogénicas durante las etapas de la vida de las carpas expuestas en la división de las células germinales (en una etapa temprana de vida y tras el desove). Se produjo feminización dependiente de la concentración de los conductos reproductivos en los peces machos cuando la exposición se producía durante las etapas tempranas de la vida, pero no en los adultos expuestos.

EVALUACIÓN DE LA EXPOSICIÓN | Exposición de la comunidad Inuit a metales trazas tóxicos
La exposición al plomo, el cadmio y el mercurio es alta en muchas comunidades Inuit del ártico debido a que los metales emitidos por las fuentes industriales y urbanas se transportan por la atmósfera hasta regiones remotas y aparecen en la alimentación local. Los sistemas domésticos de tratamiento de residuos que acumulan metales en los sedimentos ofrecen un registro histórico preciso de los cambios en la ingestión o la inhalación. Hermanson y Brozowski (p. 1308) han recogido sedimentos de un lago ártico usado para el tratamiento de residuos a fin de identificar la historia de la exposición al Pb, al Cd y al Hg. La ingestión diaria de Pb por persona en 1990 superó el nivel de ingesta diario tolerable (IDT). Los datos de Cd y Hg mostraban que su ingesta estaba por debajo de los niveles de IDT respectivos en esa fecha.
POLIMORFISMO GENÉTICO | El polimorfismo de la ALAD y la susceptibilidad a la toxicidad del plomo
Chia et al. (p. 1313) han examinado seis polimorfismos de un solo nucleótido (P1N) en la -deshidratasa del ácido delta-aminolevulínico (ALAD) para determinar si estos P1N podrían modificar la relación entre el plomo en la sangre (PbS) y los parámetros renales en un estudio transversal en trabajadores expuestos al plomo en Vietnam. Los autores aplicaron un análisis de covarianza con interacción de PbS P1N para examinar el efecto cambiante de los P1N en la asociación de los parámetros renales con el PbS, con ajustes para los factores potenciales de confusión. El P1N HpyCH4 parece modificar la toxicidad del plomo en el riñón, siendo el alelo salvaje más susceptible que las variantes.
TOXICOLOGÍA | Biomarcadores en una población consumidora de pescado
Una población cercana al río San Lorenzo en la que se consume mucho pescado muestra una gran cantidad de bifenilos policlorados (PCB) y compuestos similares a las dioxinas (CSD) en el cuerpo. Ayotte et al. (p. 1318) han medido los biomarcadores de inducción de la enzima hepática y la relación con la cantidad de organoclorados en el cuerpo de voluntarios adultos de este pueblo. No se pudieron correlacionar ni las concentraciones de CSD ni de PCB-153 (que representan los PCB totales) con los biomarcadores de efectos. A pesar de la gran cantidad de PCB y CSD en el cuerpo en esta población, sólo el tabaquismo mostró una correlación significativa con los biomarcadores de inducción de la enzima hepática. Los datos concuerdan con la actividad de la CYP1A2 hepática inducida por el tabaco que altera el metabolismo del heme y aumenta la biotransformación de los congéneres mono-orto del PCB.
SALUD DE LA POBLACIÓN | El consumo de pescado y el conocimiento de las recomendaciones
Más de 61 millones de adultos viven en los ocho estados de EE. UU. que bordean los Grandes Lagos. Imm et al. (p. 1325) han realizado una encuesta a adultos que viven en los estados de los lagos para evaluar el consumo de pescado capturado mediante la pesca deportiva o adquirido en comercios y su conocimiento de las recomendaciones estatales sobre consumo. Los resultados indican que es probable que la exposición a los contaminantes persistentes hallados en el pescado de los Grandes Lagos se limite a un segmento relativamente pequeño de la población muy consumidora de pescado capturado mediante pesca deportiva. Estos resultados también destacan la importancia de las recomendaciones sobre consumo de pescado, ya que se estima que 2,9 millones de adultos de la zona pueden encontrarse en riesgo de superar la dosis de referencia de consumo de metil mercurio, bifenilos policlorados y otros contaminantes que se bioacumulan.
CARDIOVASCULAR | Mejora de la vasoconstricción inducida por el arsénico
La exposición crónica al arsénico provoca enfermedades cardiovasculares como la hipertensión. Lee et al. (p. 1330) han evaluado si el arsénico afecta a la contracción de los anillos aórticos en un sistema de baño de órgano aislado. El tratamiento con arsenito, una especie inorgánica trivalente, aumentó la vasoconstricción inducida por fenilefrina o serotonina de manera dependiente de la concentración. Se correlacionó la hipercontracción por arsenito con la extensión de la fosforilación de las cadenas livianas de la miosina (CLM) estimulada por la fenilefrina. Otros resultados sugieren que el arsenito incrementa la vasoconstricción inducida por el agonista modulada por la fosforilación de las CLM en los músculos lisos y que la sensibilización del calcio es uno de los mecanismos clave para la hipercontracción inducida por el arsenito en los vasos sanguíneos.
EVALUACIÓN DE LA EXPOSICIÓN | Niveles elevados de perc en barrios habitados por minorías con pocos recursos
Las emisiones de tetracloroetileno (PCE, perc) de las secadoras que funcionan en los edificios de apartamentos pueden contaminar el aire del interior de las residencias. McDermott et al. (p. 1336) han determinado los niveles de perc en el aire de interior durante el periodo 2001-2003 en 65 apartamentos situados en 24 edificios de la ciudad de Nueva York, en los que las secadoras utilizaban perc. El nivel medio por apartamento de perc fue 10 veces menor que los niveles medios documentados antes de 1997. A pesar de estos logros, los niveles de perc en 17 de los apartamentos seguían superando los recomendados para el aire residencial por el Departamento de Salud del Estado de Nueva York. Los niveles medios de perc en el aire de interior en los barrios habitados por minorías son cuatro veces mayores a los encontrados en los otros barrios.
EVALUACIÓN DE LA EXPOSICIÓN | Fuentes de plomo en el cacao y el chocolate
Rankin et al. (p. 1344) presentan información sobre las concentraciones de plomo y las composiciones isotópicas obtenidas en análisis de semillas, cáscaras y suelos de cacao en seis plantaciones de cacao de Nigeria y en análisis de productos de cacao y chocolate manufacturados. La concentración media de plomo en las semillas de cacao fue de ≤ 0,5 ng/g, lo que supone uno de los valores más bajos que se conocen en un alimento natural. Las concentraciones de plomo en los productos manufacturados con cacao y chocolate se incrementaban hasta los 230 ng/g y 70 ng/g respectivamente, lo que resulta consistente con los estudios que indican que las concentraciones de plomo en los productos derivados del chocolate se encuentran entre las más altas registradas en los alimentos. La mayor parte de la contaminación se produce durante el embarque y durante el procesado de las semillas de cacao y la manufactura de los productos derivados del cacao y el chocolate.

CARDIOVASCULAR | Efectos apoptóticos cardiacos en los fumadores pasivos
Aunque el humo de tabaco ambiental (HTA) se asocia con las enfermedades cardiacas de los no fumadores, no se conoce el mecanismo molecular de la patogénesis. Kuo et al. (p. 1349) han observado que la exposición al HTA en varias dosis durante 30 minutos dos veces al día durante 1 mes produce una reducción que dependiente de la dosis en la relación entre el peso del corazón y el peso corporal y aumenta la fibrosis intersticial. Los resultados de otros análisis de expresión génica indican que los efectos del HTA en los cardiomiocitos son dependientes de la vía de muerte celular aopotótica dependiente de la activación del FAS ligando-receptor y podrían estar relacionados con la incidencia de enfermedades cardiacas en sujetos no fumadores expuestos al HTA.
TOXICOGENÓMICA | Perfiles de expresión génica y la toxicidad en la tiroides de las ratas
Los yoduros orgánicos producen hipertrofia tiroidea y aumentan las alteraciones del coloide tiroideo en las ratas. Glatt et al. (p. 1354) han evaluado el efecto del yoduro libre (como el yoduro de sodio) en la toxicidad tiroidea en comparación con el fenobarbital (FB) y el propiltiuracil (PTU). Los análisis de expresión génica de la tiroides mediante el equipo U34A GeneChips de Affymetrix, una prueba t normalizada, y los sistemas Gene Map Annotator y Pathway Profiler demostraron cambios significativos en las transcripciones de receptor acoplado a proteína G de tipo rodopsina en todos los productos químicos. El NaI muestra cambios dependientes de dosis en varios genes oxidativos relacionados con el estrés. Se observaron perfiles de trascripción diferencial, que posiblemente sean relevantes para el desarrollo de células tumorales foliculares, en ratas expuestas al FB y al PTU, incluidos genes implicados en la señalización Wnt y la expresión de las proteínas ribosomales.
DESARROLLO FETAL | El riesgo de padecer anomalías congénitas y los vertederos
En Gales están preocupados sobre la posibilidad de que vivir cerca de un vertedero haya producido un aumento en el riesgo de nacimientos con malformaciones congénitas. Palmer et al. (p. 1362) han evaluado si los residentes que viven cerca de 24 vertederos de Gales experimentan un aumento en la tasa de anomalías congénitas tras la apertura del vertedero en relación a los periodos anteriores. La relación de las tasas observadas y las esperadas de anomalías congénitas antes de que se abrieran los vertederos es de 0,87 y aumentó al 1,21 tras la apertura, lo que da una tasa estandarizada de riesgo de 1,39. Las deducciones causales son difíciles, ya que no se ha podido establecer por completo el caso, faltan datos de exposiciones a nivel individual y de otros factores socioeconómicos y de estilo de vida que podrían suponer un factor de confusión respecto al área de residencia.
Medicina medioambiental IDENTIFICACIÓN DE RIESGOS | Estudio de exposición y respuesta al berilio
Rosenman et al. (p. 1366) han llevado a cabo una exploración médica en busca de enfermedades relacionadas con el berilio en antiguos trabajadores de una instalación de procesado de berilio. La prevalencia de beriliosis crónica probada o probable fue del 7,6 %, y otro 7,0 % de los casos mostraban sensibilización al berilio. El nivel de beriliosis crónica y de sensibilización en el grupo de estudio era mayor que el indicado en los estudios de otros grupos expuestos al berilio. Tanto la beriliosis crónica como la sensibilización se produjo en antiguos trabajadores cuya exposición laboral diaria media a lo largo de toda su vida fue menor que los niveles actuales en el aire en el lugar de trabajo permitidos por la Administración de Sanidad y Seguridad en el Trabajo, 2 µg/m3, y por la recomendación del Departamento de Energía, 0,2 µg/m3.
ENFERMEDADES INFECCIOSAS | Reaparición de la varicela ocupacional
Algunas apariciones comunes de las infecciones por el virus de la varicela zóster (VVZ) son la varicela (primaria) y el herpes (recurrente o latente), o culebrilla, así como una inmunidad permanente contra la varicela. Ku et al. (p. 1373) informan sobre el caso de una enfermera profesional que trabajaba en un centro de cirugía neurológica en un hospital general de Taipei, Taiwán. La enfermera desarrolló varicela recurrente tras tratar a un paciente con paraplejía que sufrió de herpes zóster durante su hospitalización en el 2002. Los análisis negativos para IgM específica y positivos para IgG específica en suero indicaron una infección por VVZ en el pasado. Existe peligro de infección ocupacional por VVZ en ambientes de atención sanitaria; por lo tanto, se recomienda realizar pruebas del anticuerpo del VVZ y un programa de vacunación contra el VVZ para aquellos trabajadores sanitarios susceptibles.
Salud infantil DESARROLLO FETAL | El pescado, el mercurio y la cognición infantil
El pescado y otros alimentos marinos pueden contener mercurio orgánico, pero también nutrientes beneficiosos como los ácidos grasos poliinsaturados n-3. Oken et al. (p. 1376) han examinado las asociaciones entre el consumo de pescado en mujeres durante el embarazo y el mercurio materno en el pelo en el momento del parto con la cognición infantil en pares de madres e hijos. La cognición infantil se probó mediante el porcentaje de preferencia por la novedad en la memoria de reconocimiento visual (MRV) a los 6 meses de edad. Un consumo mayor de pescado se asoció con una mayor cognición infantil. No obstante, un incremento de 1 ppm de mercurio se asoció con un descenso de la puntuación de MRV. Las mujeres deben seguir comiendo pescado durante el embarazo, pero deben elegir las variedades con menor contaminación de mercurio.

EVALUACIÓN DE LA EXPOSICIÓN | El metil mercurio y el mercurio inorgánico durante la lactancia
Se sabe que el metil mercurio (MeHg) y el vapor de mercurio atraviesan la barrera placentaria, pero no se conoce mucho sobre la exposición infantil a través de la leche materna. Björnberg et al. (p. 1381) han medido el MeHg y el mercurio inorgánico (Hg-I) en la sangre de madres y sus hijos, y el mercurio total (Hg-T) en la leche materna durante un periodo de 13 semanas tras el parto. El MeHg en sangre de los niños estaba asociado con el MeHg en sangre de las madres en el momento del parto y disminuyó en las 13 semanas siguientes. El Hg-I en sangre de los niños estaba asociado con el Hg-I en sangre de las madres en el momento del parto y disminuyó en las 13 semanas siguientes. La exposición a ambas formas de mercurio es mayor antes del parto, y el MeHg parece contribuir más que el Hg-I a la exposición infantil postnatal a través de la leche materna.
ESTRÉS OXIDATIVO | Asociación del estrés oxidativo del ADN con la exposición a los metales
Los efectos para la salud de la exposición al arsénico, al cromo y al níquel son confusos en el caso de los niños. Wong et al. (p. 1386) han efectuado un estudio transversal para investigar las posibles asociaciones entre las concentraciones internas de As, Cr y Ni con el estrés oxidativo del ADN en los niños. El nivel medio en la orina de 8hidroxideoxiguanosina (8OHdG) era de 11,7 ng/mg de creatinina. No se detectó ninguna relación entre el Ni en la orina y el 8OHdG. Los niños con niveles altos en la orina de Cr y As mostraron niveles mayores de 8OHdG en la orina que los que presentaban niveles bajos de estos metales. Los niños con niveles altos en la orina tanto de As como de Cr presentaron los niveles más altos de 8OHdG en la orina.
INMUNIDAD | La contaminación del aire y los fenotipos de los linfocitos del cordón umbilical
Hertz-Picciotto et al. (p. 1391) han examinado las asociaciones a corto plazo de la exposición a la contaminación del aire con los inmunofenotipos de los linfocitos en la sangre del cordón umbilical en los partos ocurridos en la República Checa. Se midió la materia particulada fina de menos de 2,5 µm de diámetro (PM2,5) y 12 hidrocarburos aromáticos policíclicos (PAH) en muestras de 24 horas. Las muestras de sangre del cordón umbilical se analizaron para determinar los fenotipos de los linfocitos T CD3+ y sus subconjuntos CD4+ y CD8+, los linfocitos B CD19+ y los linfocitos granulares grandes. Se asociaron los niveles medios de PAH o PM2,5 durante los 14 días anteriores al parto con una disminución de las fracciones del fenotipo de los linfocitos T y un claro aumento de la fracción del linfocito B. La contaminación del aire puede influir en la distribución relativa de los inmunofenotipos de los linfocitos en el feto.
EVALUACIÓN DE LA EXPOSICIÓN | Las fuentes y los factores que determinan la concentración de ftalatos en espacios cerrados
Un estudio reciente informa sobre la asociación entre los síntomas del asma y las concentraciones de nbutilbencil ftalato (BBzP) y de di(2-etilhexil) ftalato (DEHP) en el polvo recogido en las habitaciones de niños. Bornehag et al. (p. 1399) han examinado la asociación existente entre estos ftalatos con las características del polvo de las casas. Se encontraron asociaciones entre las concentraciones de BBzP y de DEHP y la cantidad de cloruro de polivinilo usado en los materiales del suelo y las paredes de la casa. Estas asociaciones pueden ayudar a identificar las casas en las que es probable que las concentraciones de ftalatos sean elevadas, y pueden ayudar a desarrollar estrategias de paliación.
ENFERMEDADES RESPIRATORIAS | Los niveles de hongos y la rinitis alérgica
La sensibilización a los hongos, como los Alternaria, se asocia con la rinitis alérgica y el asma en los niños. Stark et al. (p. 1405) encontraron altas concentraciones domésticas de hongos (mayores al percentil 90) medidas una vez en los primeros 3 meses de vida en un estudio poblacional prospectivo en el nacimiento de niños con padres asmáticos o alérgicos como predicción de rinitis alérgica diagnosticada por doctores en los primeros 5 años de vida. Las predicciones de rinitis alérgica incluían altos niveles en el polvo de Aspergilluss, Aureobasidium y levaduras. Las altas concentraciones de hongos e informes de daños por agua y moho en las casas pueden predisponer a los niños con un historial familiar de asma o alergia a que desarrollen una rinitis alérgica.
INMUNIDAD | El tabaquismo materno y la interleucina-1 de la leche materna
Zanardo et al. (p. 1410) han comparado las concentraciones de interleucina-(IL)1 y de inmunomoduladores β-endorfina y leptina en la leche transicional y de calostro en mujeres fumadoras y no fumadoras. Las concentraciones de IL-1 se reducen significativamente en el calostro de las mujeres fumadoras. Las concentraciones en el calostro de la β-endorfina y la leptina eran comparables. No se observan diferencias significativas entre las mujeres lactantes fumadoras y las no fumadoras en las concentraciones de IL-1, β-endorfina y leptina de leche transicional. Es más, las concentraciones de β-endorfina y leptina se redujeron de forma significativa en las muestras de leche transicional en comparación con el calostro en ambos grupos. El nivel alterado de IL-1 incrementa nuestro entendimiento acerca de cómo la alimentación por leche materna podría no proteger contra infecciones en los neonatos criados por madres fumadoras.

Este minimonográfico (p. 1414) pone de relieve las experiencias de los Centros de Investigación para la Prevención de Enfermedades y la Salud Medioambiental Infantiles del Instituto Estadounidense de Ciencias de la Salud Medioambiental y la Agencia de Protección Medioambiental de EE. UU., con especial interés en el Estudio nacional sobre la infancia (National Children's Study). Se incluyen problemas metodológicos generales para la realización de estudios longitudinales poblacionales de nacimientos e investigaciones participativas basadas en la comunidad y para la medición de las exposiciones a la contaminación del aire, la exposición a los pesticidas, el asma y la toxicidad neuroconductal. Esta explicación de las lecciones aprendidas ofrece una oportunidad a los planificadores del Estudio nacional sobre la infancia de utilizar la información sobre lo que ha funcionado y lo que no, al estudiar diversos grupos multirraciales y multiétnicos de niños con exposiciones urbanas y rurales únicas.
