To successfully complete the Personal Identity Verification process, applicants are required to undergo fingerprinting and complete the appropriate e-QIP background questionnaire*. Each procedure calls for the applicant to fill out a specific form or have their AO or Authorized Representative complete a form on their behalf. Applicants are strongly urged to deliver the forms or transmit them electronically per the instructions. An applicant who fails to complete the e-QIP process will have his or her badge inactivated.

It is important to understand that the background check is a Federal government job requirement. Providing information is voluntary, but if you choose not to provide the required information, you will not meet the requirements of the job and will therefore not be considered further for employment with the Federal government. If you are already employed by the Federal government, your appointment will be terminated. The courts have upheld this principle.

* Summer students are not required to undergo fingerprinting or complete an e-QIP background questionnaire unless their position merits added security measures. Summer students undergo a simple background check (NCIC) administered by the NIH Police.
