Medical Home Model for Children after Life-Threatening Illness or Injury: A National Experts' Conference

March 13-14, 2007

Location: Doubletree Hotel and Executive Business Center - Bethesda, Maryland

Sponsor/Co-Sponsor(s): Pediatric Critical Care and Rehabilitation (PCCR) program of the National Center for Medical Rehabilitation Research (NCMRR) at the National Institute of Child Health and Human Development (NICHD), in conjunction with the American Academy of Pediatrics (AAP) and the Health Resources and Services Agency (HRSA).

Purpose: A multidisciplinary group of experts will critically evaluate the existing literature on implementation and effectiveness of the medical home model for children with special health care needs, specifically targeting children who have developed chronic healthcare needs as an outcome of critical illness or injury. Participants will critique existing scientific data, identify gaps in research, and identify scientific initiatives or investigations that would be helpful for addressing these research needs to benefit these children and their families.
