If you are an academic researcher, follow these steps to obtain the materials you are requesting:

  3. Sign the document as Recipient Scientist AND obtain the signature of your institution's Authorized Signatory, where indicated. (Or, if you are a student or postdoctoral fellow, have your advisor or supervisor sign as Recipient Scientist.)

  4. Fax or e-mail a pdf version of the document signed by both you and the institution's signatory to Rochelle Blaustein at

  5. Upon our receipt of the properly signed, unmodified document, we will authorize transfer of the materials by the Provider Scientist (this is why it is important that you provide that information). We will return a scanned, color Adobe pdf version of the fully executed document to you and your technology transfer professional at the e-mail addresses you have provided.

    IF ANY MODIFICATIONS TO THESE DOCUMENTS ARE NEEDED BY YOUR INSTITUTION, HAVE YOUR TECHNOLOGY TRANSFER PROFESSIONAL DOWNLOAD THE MS WORD VERSION OF THE APPLICABLE DOCUMENT AND SEND A REDLINED, UNSIGNED VERSION REQUESTING THE MODIFICATIONS BY E-MAIL ATTACHMENT TO ROCHELLE BLAUSTEIN AT Rochelle.Blaustein@nih.gov SO WE MAY CONSIDER YOUR REQUEST.
