Items Required for Initial Evaluation by the Pediatric
Oncology Branch
The patient/family should either bring the following items or have the
home hospital/physician express them to the Attending Physician prior
to the first visit to the Pediatric Oncology Branch. The referring physician
will receive a list of the required items. Pathology
- Pathology slides and a pathology report must be available for any
biopsy or surgical procedures performed by the home hospital. A repeat
biospy may be required if additional material is necessary to reach
a conclusive diagnosis.
Radiological Evaluation
- For newly diagnosed patients, a copy of all radiological scans, to
include x-rays, CT scans, MRI scan, and nuclear medicine scans (e.g.,
bone scan)
- For children with recurrent cancer, a copy of the two most-recent
sets of scans and x-rays.
Clinical Summary
- The referring physican should write a letter summarizing
- the patients initial presentation.
- diagnostic studies performed.
- treatment (if any) administered, including chemotherapeutic agents
used and radiation doses employed.
Laboratory Evaluation
- A copy of the most-recent laboratory studies
