SUBMIT AN APPLICATION:
If you are already an NIH CME Activity Director, just Login at
left,to be taken to your Activity Director home page.
If you are new to the CME program, please complete and submit a
New Activity Information Form (PDF).
Applications less than 12 weeks out from the activity start
date are required to consult with the NIH office at 301/496-9425 prior to
submitting the online application. In addition, under the very rare
circumstance that the NIH CC office approves moving forward with application
review and approval, you should expect the process to take 12 weeks and
therefore the start date for the activity could be significantly impacted.

completed spreadsheet to the CME Office
