Identification Card

All employees, guest researchers, certain categories of contractors, and volunteers are required to have an NIH (DHHS) Identification Card (HHS 576) and are required to display the Identification Card (ID) while on the NIH campus or in NIH buildings or leased facilities.

To obtain an identification card, ask your administrative/personnel office to complete Form NIH-1308-4, Identification Card Application. Take this form, along with a valid photo identification (driver's license, passport, or college ID), to the NIH Parking Office. Cards are issued Monday through Friday from 8:30 a.m. to 4:30 p.m.

You are responsible for safeguarding your card. If your card is lost or stolen, please notify your administrative/personnel office immediately. Identification cards are the property of the U.S. Government and must be returned to the Parking Office upon transfer to another location or termination of employment.

If you transfer to another NIH institute you will need to obtain a new ID card. Before obtaining the new ID card, you must return your current one to the Personnel Office of the institute you are transferring from.

You must surrender your ID card to the personnel office of your institute when you leave the NIH permanently.
