NIH Census Request Form

To obtain access to the NIH Census web site, you must first have a valid IC Domain network account, which you will use to logon to the Census Application. You must provide all information requested below. When you have filled in all blanks, click on the button titled "Submit Request". Your request will be routed to the Office of Facilities Planning; you will be notified when OFP has approved your request.

Your Census Role:	IC Census Certifier or IC Census Taker (see below)
To choose your Census Role, select IC Census Certifier if you are in a position within your IC to approve all census data, normally the Executive Officer. Select IC Census Taker if you are the person assigned to enter census information for a particular organization, normally the Administrative Officer.
