Survey Instruments

In addition to facilitating access for researchers to data resources and reporting the results of the following studies, we make available the survey instruments for the studies.

Health Services & Economics Branch Instruments

• National Survey of Colorectal Cancer Screening Practices - A national assessment of physician knowledge, attitudes, and practices and health plan programs and policies concerning colorectal cancer screening to help inform colorectal cancer control efforts.
• Patterns of Care / Quality of Care Studies - Studies that began in 1987 to evaluate the dissemination of state-of-the-art cancer therapy into community practice.
• Prostate Cancer Outcomes Study (PCOS) - The first population-based evaluation of health-related quality-of-life issues for prostate cancer patients conducted on a multi-regional scale.

Applied Research Program Instruments

• National Health Interview Survey (NHIS) Cancer Control Module - A nationwide survey of about 36,000 households in the U.S. and a principal source of information on the health of the civilian noninstitutionalized population.
• California Health Interview Survey (CHIS) - A telephone survey that provides population-based, standardized health-related data from 55,000 households selected from all 58 counties in the state.
