A new web-based application—the Admission/Travel/Voucher Request System—has taken over many tasks that used to be handled by the MIS Form 54.
Use the ATV system (http://atv.cc.nih.gov)to request:
- Patient admission
- Patient demographic changes
- Assignment of patients to a new protocol for a new admission
- Assignment of patients to a new physician for a new admission
- Government-sponsored patient travel (arrangements made by the Patient Travel Office)
- Vouchers for patient travel
- Vouchers for lodging and meals
