Form 54 Moves to the Web

 

A new web-based application—the Admission/Travel/Voucher Request System—has taken over many tasks that used to be handled by the MIS Form 54.

Use the ATV system (http://atv.cc.nih.gov)to request:

  • Patient admission
  • Patient demographic changes
  • Assignment of patients to a new protocol for a new admission
  • Assignment of patients to a new physician for a new admission
  • Government-sponsored patient travel (arrangements made by the Patient Travel Office)
  • Vouchers for patient travel
  • Vouchers for lodging and meals

The new application was designed to be straightforward to use, and has extensive online help (click "Help" on the ATV web page). If you need additional assistance using the application, please contact one of the following (these contacts are also listed in the ATV online help):

  • For questions about LOGGING ON or other computer technical issues: CIT Help Desk, 301-496-4357
  • For questions about ADMISSION requests, patient demographic updates, or protocol and physician changes: CC Admissions, 301-496-3141 (24 hours a day/7 days a week)
  • For questions about patient TRAVEL VOUCHER requests: the Voucher Office, 301-496-7632
    (8 to 5 Monday through Friday except holidays)
  • For questions about patient TRAVEL ARRANGEMENTS: the Patient Travel Office, 301-496-6676
    (24 hours a day/7 days a week)
  • For other ADMINISTRATIVE questions: Ambulatory Care Services, 301-496-2341
    (7 to 5 Monday through Friday except holidays)
