Form DS-5504: Name Change, Data Correction, and Limited Passport Book Replacement


SUBMIT FORM DS-5504 IF:

  • You need to change your name in your valid U.S. passport
  • You need to extend the validity of a limited passport (if eligible)
  • You need to correct a printing error in your valid U.S. passport

If your U.S. passport was issued more than one year ago, you may not be eligible to use Form DS-5504. See Correcting or Changing Information in Your U.S. Passport for more information.

STEPS TO SUBMITTING FORM DS-5504 :

Read and understand Steps 1-3 before leaving this page. There is no fee when submitting Form DS-5504 using Routine Processing. Printing errors (typos) will be corrected at any time while the passport is valid for no fee using Form DS-5504.

STEP 1: Complete and Print Form

  • If completing Form DS-5504 online, make sure that all of the entered information is complete and correct before printing. Print the completed form using black ink on one-sided pages.

  • If completing Form DS-5504 by hand, do so legibly and in black ink.  Make sure that all of the entered information is complete and correct. Illegible applications may delay application processing times.

NOTE: Form DS-5504 can also be obtained from your local Acceptance Facility or a Passport Agency

STEP 2: Sign and Date

You must sign and date Form DS-5504 on page 1 of the application.

STEP 3: Submit Completed Form + Additional Documents

  • If it has been less than 90 days since your passport was issued, please mail Form DS-5504 to the address listed on the form, along with:
  • Your valid U.S. passport
  • Evidence to support the change requested (see Instruction Page 2 of the form)

NOTE: We strongly encourage applicants to use a mail service which provides delivery confirmation when mailing a passport application or any additional documentation to the Department of State

  • If it has been more than 90 days since your passport was issued, please mail Form DS-5504 to the address listed on the form, along with:
  • Your valid U.S. passport
  • Evidence to support the change requested (see Instruction Page 2 of the form)
  • Two new Passport Photos

If You Wish to Expedite Your Passport Application:

  • Include a check or money order for $60.00 made payable to the U.S. Department of State (See Passport Fees)
  • Clearly mark ‘EXPEDITE’ on the outside of the envelope
  • We recommend using an overnight delivery service for delivery to and from the Passport Agency

NEED A U.S. PASSPORT IMMEDIATELY?

You should make an appointment to be seen at a Regional Passport Agency only if:

  • The U.S. passport is needed in less than 2 weeks for international travel
  • The U.S. passport is needed within 4 weeks to obtain a foreign visa
