Agency Information Collection Activities: Proposed Request and 
Comment Request

    In compliance with Public Law 104-13, the Paperwork Reduction Act 
of 1995, SSA is providing notice of its information collections that 
require submission to the Office of Management and Budget (OMB). SSA is 
soliciting comments on the accuracy of the agency's burden estimate; 
the need for the information; its practical utility; ways to enhance 
its quality, utility and clarity; and on ways to minimize burden on 
respondents, including the use of automated collection techniques or 
other forms of information technology.

    I. The information collections listed below will be submitted to 
OMB within 60 days from the date of this notice. Therefore, comments 
and recommendations regarding the information collections would be most 
useful if received by the Agency within 60 days from the date of this

    1. Request for SSI Benefit Estimate--0960-0492. The Social Security 
Administration (SSA) uses Form SSA-3716 for Supplemental Security 
Income (SSI) beneficiaries who wish to request a 5-month estimate of 
what their benefits would be if they should return to work in the 
future. The respondents are SSI Recipients or Applicants for SSI 
Benefits.
    Number of Respondents: 50,000.
    Frequency of Response: 1.
    Average Burden Per Response: 5 minutes.
    Estimated Annual Burden: 4,167 hours.
    2. Supplemental Statement Regarding Farming Activities Of Person 
Living Outside The U.S.A.--0960-0103. Form SSA-7163A-F4 is used by SSA 
to collect needed information whenever a Social Security beneficiary or 
claimant reports work on a farm outside the U.S. The information is 
used to make a determination for work deduction purposes. The 
respondents are Social Security beneficiaries or claimants who are 
engaged in farming activities outside the U.S.
    Number of Respondents: 1,000.
    Frequency of Response: 1.
    Average Burden Per Response: 60 minutes.
    Estimated Annual Burden: 1,000 hours.
    II. The information collection listed below has been submitted to 
OMB for clearance. Written comments and recommendations on the 
information collection would be most useful if received within 30 days 
from the date of this publication. Comments should be directed to the 
SSA Reports Clearance Officer and the OMB Desk Officer at the addresses 
listed above. You can obtain a copy of the OMB clearance package by 
calling the SSA Reports Clearance Officer on (410) 965-4145, or by 
writing to him.
    Pre-1957 Military Service Federal Benefit Questionnaire--0960-0120. 
Form SSA-2512 collects data used in the claims adjudication process to 
grant gratuitous military wage credits, when applicable, and solicits 
sufficient information to make a determination of eligibility. The 
respondents are individuals who are applying for Social Security 
benefits on the record of a wage earner with pre-1957 military service.
    Number of Respondents: 12,000.
    Frequency of Response: 1.
    Average Burden Per Response: 10 minutes.
    Estimated Annual Burden: 2,000 hours.
