The definition of the score variable CIQDSCOR has been corrected in the documentation. 
The algorithm to compute scores has been corrected in the appendix. Records for people
in the sub-sample but not participating in CIDI have been added have been added to the 
file. Four observations have corrected values, and an additional thirty observations 
have data for previously missing values.
