Program Locations
Missouri
Background: Since 2003, the Missouri Department of Health and
Senior Services has been funded as a standard project.
Lifestyle Intervention: Missouri will use A New Leaf…Choices for
Healthy Living to help women develop a healthier diet, increase
physical activity, and be tobacco free. New Leaf is a 3–session
counseling tool that emphasizes individual tailoring, goal&ndsh;setting, social
support, and identification of barriers and perceived benefits.
Screening: Risk factor screening includes blood pressure, glucose,
cholesterol and other lipids testing as well as assessing weight, medical
history, tobacco use, poor diet, and physical inactivity.
Sites: The addition of cardiovascular disease services to the Show
Me Healthy Women intervention will be piloted at 5 sites in the Kansas
City and St. Joseph area. The program will then be expanded to include
federally qualified community health centers and other Breast and Cervical
Cancer Early Detection Program providers throughout the state.
Key partners: Missouri’s Local Public Health Agencies, the Department of
Social Services, Missouri Heart and Stroke Program, Missouri Diabetes
Prevention and Control Program, and the American Heart Association.
