Spotlight On Partner Activities
Charles B. Wang Community Health Center Aims to Improve Diabetes Care and
Health Literacy of Patients with Diabetes
The
Charles B. Wang Community Health Center has joined
the New York City Health Department Diabetes Collaborative Care initiative. The
health
center has developed a patient registry and is tracking clinical outcomes to
monitor and improve diabetes care. Currently, they have approximately
800 active patients with diabetes in the system. The health center is
developing new materials for health education and self-management support for
the Chinese community it serves.
National Hispanic Medical Association Helps Launch New MOVIMIENTO, Por Su Vida CD
Hispanic/Latino Work Group Vice-Chair Dr. Jaime Torres, associate director of Consultative
Services, Coler-Goldwater Specialty Hospital, who represents the National Hispanic
Medical Association on the Work Group, helped launch the awareness campaign
promoting NDEP’s new MOVIMIENTO, Por Su Vida (Movement, For Life) music CD.
Taking on Diabetes
The American Association of Health Plans (AAHP) is a national trade
association representing more than 1,000 health maintenance organizations,
preferred provider organizations, point-of-service plans, and other
similar health plans that care for more than 140 million Americans. AAHP
and the American Diabetes Association (ADA) have partnered to create,
Taking on Diabetes, a joint initiative of the AAHP and the ADA that
brings together health plans, health professionals, and people with
diabetes in order to reduce the complications associated with diabetes.
