Heart Disease Death Rates for 2000 through 2004 of Adults Aged 35 Years and Older by County. The map shows that concentrations of counties with the highest heart disease rates – meaning the top quintile – are located in Appalachia, along the southeastern coastal plains, inland through the southern regions of Georgia and Alabama, and up the Mississippi River Valley.
