RCI Honors Georgia's Caregivers of the Year Care Transition Coach Twenty-five individuals have been trained to serve as Care Transition Coaches at a recent training in Athens, Georgia. Training was led by Dr. Eric Coleman and colleagues from the University of Colorado Denver. The Care Transitions Intervention is an Evidence-Based program designed to empower patients and caregivers to effectively manage transitions between care setting. The training was sponsored by RCI and CARE-NETS as part of the Georgia Coalition for Caregiver Health. Read more about Care Transitions. Georgia Coalition to Promote Caregiver Health Rosalynn Carter Institute and its partner CARE-NETs have launched a new state-wide Advocacy organization dedicated to promoting caregiver health in Georgia. The Coalition will work to establish state-wide training to equip caregivers with the skills they need to manage their health, deal with high levels of stress, and solve caregiving challenges.
