These problems are all with the previous version,

and are corrected in:

Are you using the schema at
http://www.loc.gov/standards/mads/mads-preliminary-draft-2-dec-17.xsd

I can't find any instances of  'HierarchicalGeographic'  or 'IdentifierType'
