简介
有关工作时间与健康之间关系的研究一直都没有定论。因此,我们努力考查工作时间对动脉粥样硬化进展的影响。

方法
此项前瞻性研究调查了 621 名中年芬兰男子,评估了工作时间的基线及重复测量值对超音测定的颈动脉内膜中层厚度 (IMT) 11 年进展的影响及与心血管病的相互作用。多个线性回归模型经过 21 项生物学、行为学和社会心理学风险因素的调整。

结果
在基线水准,每周工作 3 天 (最少)、5 天 (中等) 或 7 天 (最多) 分别与 IMT 在 11 年中增厚 23%、31% 和 40% 相关。按每周的基线工作天数,最多天数与最少天数的相对变化率 (RCR) 为 1.14,而按随访年的每年工作小时数,该比率为 1.10。心血管病与工作时间之间存在显著的相互影响。患缺血性心脏病 (IHD) 的男子,每天最多工作 14.5 小时者,IMT 增厚 69%,而没有 IHD 的男子则增厚 29%。按每天的小时数,IHD 的 RCR 比率 (RCRIHD/RCRno IHD) 是 1.44。同样,按每天的小时数,基线颈动脉狭窄的 RCR 比率是 1.29,而按每年小时数,则该比率是 1.22。

结论
在中年男子中,尤其是在原先患有心血管病的中年男子中,工作时间的增加与颈动脉粥样硬化呈正向相关。我们的发现与粥样动脉硬化的血液动力学理论一致。
