RMKS/ 1. THE REQUIREMENT TO CONDUCT A FORMAL SAFETY INVESTIGATION
FOR ALL MISHAPS INVOLVING EXPLOSIVE ORDNANCE (ALL TYPES), REGARDLESS
OF EXTENT OF INJURIES OR DAMAGE, REMAINS VALID. FSIR'S REQUIRE
ENDORSEMENTS FROM COMMANDS IDENTIFIED BY THE FORCE COMMANDER AS
MEMBERS OF THE ENDORSING CHAIN OF COMMAND WITH APPLICABLE COMMENTS.
CMC(SD) OCCASIONALLY RECEIVES VERBAL REPORTS OF INJURIES INVOLVIONG FIREARMS AND ORDNANCE, YET DOES NOT SEE THE FSIR. MISHAPS INVOLVING
ORDNANCE ITEMS WHERE INJURY IS NOT SERIOUS OR EVEN SUPERFICIAL MAY
INDICATE TRENDS OR THE PRESENCE OF CAUSAL FACTORS WHICH MAY BE
DEADLY IN SIMILAR SITUATIONS.
2. FSIRS NEED TO BE SUCCINCT FOR EASE OF REPORTING AND ENDORSING.
AVOID THE 'NOVEL' APPROACH. BULLET FORMAT MOST PREFERRED IN THE
FOLLOWING PARAGRAPHS OF THE FSIR:
A. FINDINGS: PARA 7 OF THE FSIR. TOO MUCH REFERAL TO SUPPORTING
DOCUMENTS, PERSONAL INTERVIEW, SITE INVESTIGATION AND ENGINEERING
INVESTIGATION ETC. FINDINGS MUST LEAD REVIEWER THROUGH A SEQUENCE
OF EVENTS AS TO WHAT HAPPENED.
B. ANALYSIS OF EVIDENCE: PARA 8 OF THE FSIR. REVIEW ALL EVIDENCE
AND WRITE A BULLET ABOUT EACH AS TO WHAT IT PROVIDED, IF ANTHING.
C. CONCLUSION: PARA 9 OF THE FSIR. SEPARATE THE CONCLUSION INTO
TWO GROUPS: CAUSAL FACTOR(S) OF THE MISHAP, AND CAUSAL FACTOR(S)
OF INJURIES AND PROPERTY DAMAGE, ALL IN BULLET FORMAT. CONCUR
OR NON CONCUR WITH EACH.
D. RECOMMENDATIONS: PARA 10 OF THE FSIR. FOR EACH CONCLUSION,
WRITE A RECOMMENDATION IDENTIFYING WHO IS TO DO WHAT, AND WHEN IT IS
TO BE COMPLETED. CONCUR OR NON CONCUR WITH EACH.
3. THE ABOVE PROCEDURES ARE INCLDUED IN THE REVISION OF REF A
WHICH IS CURRENTLY IN THE STAFFING PROCESS AT HQMC. THIS MESSAGE
