Speaker Bio
Nicole Smith, PhD, MPH, MPP, joined the Centers for Disease Control and Prevention (CDC) as a research assistant on a CDC project promoting the birth dose of hepatitis B vaccine, and later she became an Association of Schools of Public Health Fellow in the Hepatitis Branch at the National Center for Infectious Diseases (NCID). She became an Epidemic Intelligence Service (EIS) Officer with the Health Studies Branch in the National Center of Environmental Health in 2001. Following EIS, Nicole returned to the National Immunization Program where she was the Associate Director for Policy in the Immunization Services Division. In November 2005, Nicole joined the NCID/Influenza Branch where she is working on domestic pandemic preparedness efforts while also assisting the Advisory Committee on Immunization Practices regarding the prevention and control of influenza.

Hurricane Morbidity Report Form: For Active Surveillance in Clinical Care Settings. This interim form is for individual-level active surveillance of medical conditions of public health importance after hurricanes. Clinicians should use the one-page form to record information about the chief complaints and specific infectious syndromes, mental health conditions, injuries, and chronic diseases that best describe the reason the patient is currently seeking care.
