We were recently notified by Part B News that the 855 forms that we have made available on the CMS website are no longer capable of being saved by a user who is using Adobe Reader. |

April 28: We verified the information and then worked with CMS staff to upgrade the forms. They can once again be saved by a user even if the user does not have access to the full Adobe package.
