Issue The 855 form is now available on the CMS website in a form that can be filled out but we have learned that unless the doctor has the full Adobe suite the completed form can not be saved.

We are working on a fix for this problem.

November 9: CMS is in discussion with Adobe with the intentions of allowing physicians to save completed 855s even if they do not own the full Adobe Acrobat suite.

January 22: We expect to have a form which physicians can complete and save even if they do not own the full Adobe suite within 90 days.

February 13: An 855 form which can be filled out and saved on any computer should be available by early June.

July 24: The 855 forms available on the CMS website can now be saved even if the user does not have the full Adobe Acrobat suite. This will allow the user to complete part of the form, save it, and then come back to complete it at a later date.

May 19: Online enrollment has been delayed while we work on security issues.
