How do I get an appointment?
A patient already assigned to a primary care manager (PCM) will need to call 719-457-2273. If you are being referred to IMC from another provider, we will receive a referral and someone will contact you to set up an appointment.
How do I get my medication refilled/renewed?
Variety of ways:
- Simply calling the medication refill line: 719-333-3784. Be sure to have your prescription number (located on the label) with you.
- If you're told that you're medical refill has expired, contact the clinic to verify if an appointment is needed to have a new prescription written or if your provider will be able to enter a new request.
You will need to fill out & sign a medical release of information form with the Outpatient Records section or you may sign a similar release form at the provider's office. From the provider's office you may fax it to us. The fax number is: 719-333-4998
