  • Scheduled appointments with Primary Care Manager (PCM)
  • Walk in services (blood pressure checks, suture removal, throat cultures) available Monday through Friday 0800-1100 & 1300-1500
  • You may request to be scheduled in any of the 10 MDG patient education classes (diabetes education, high cholesterol/high blood pressure education, or asthma education) by calling 457-CARE. No referral is needed.
  • Behavioral health services are offered within the Family Practice Clinic and can be scheduled by calling the appointment line (457-2273). The behavioral health consultant works with you and your PCM to address behaviors, thoughts, emotions, stress, and relationships that may impact your physical health and quality of life.

      How do I schedule an appointment?

If you are assigned a Family Practice PCM, you simply need to call 457-CARE and follow the voice prompts to make an appointment with your PCM.

      How do I get a refill on medication?

Although you can use the automated system for medication refills, any medication that needs to be reordered or changed must go through your PCM Team. This is done by calling 457-CARE and scheduling an appointment with your PCM. If you need a temporary renewal on your medication (90 days or less) until you can get in for an appointment with your PCM, you can schedule an appointment with the pharmacy prescription renewal clinic. This is also done by calling 457-CARE.

      How do I obtain copies of my medical information?

A signed medical release of information will be sufficient. These forms can be obtained from the Admissions and Dispositions section.

      How do I obtain a Pregnancy Test/OB consult?

Request a test at the GYN Clinic, fill out the information sheet, and proceed to the lab. A nurse will call you in the afternoon if it is positive. At that time a consult will be placed for OB care with the TriCare Contractor.

      What should I do if I am unable to keep a scheduled appointment?

In order to maintain available appointments for all of our patients, we request that you call 457-CARE to cancel your appointment. By you canceling your appointment, we will be able to provide timely services to our other beneficiaries.
