Program Description
VA Home Based Primary Care is a unique home care program that provides comprehensive, interdisciplinary, primary care in the homes of veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective. In contrast to other home care systems that target patients with short-term remediable needs and provide episodic, time-limited and focused skilled services, HBPC targets patients with complex, chronic, progressively disabling disease and provides comprehensive, long term home care. HBPC is designed to serve the chronically ill through the months and years before death, providing primary care, palliative care, rehabilitation, disease management and coordination of care services.
General Program Requirements
To be eligible for Home-Based Primary Care, you must be enrolled in the VA health care system or be eligible for VA health care without the need to enroll. In addition, several factors are considered in determining whether it would be appropriate to admit you to the HBPC program. These include: - Having a complex, chronic disabling disease that necessitates care by an interdisciplinary team
- High risk for recurrent hospitalization or nursing home placement
- Determination by the HBPC team that the home is the most appropriate venue for care
