Primary Care Case Management: Physicians Invited
  Please note: the deadline for PCCM Provider Enrollment has been extended to October 31, 2008. DSS is inviting Connecticut primary care physicians to consider enrolling in the Primary Care Case Management (PCCM) pilot program for HUSKY A members. This new health care delivery model is explained in the program plan and request for applications.
