GENTEXT/REMARKS/1. AS PART OF THE HQMC SAFETY DIVISION'S
ONGOING EFFORTS TO PROVIDE INFORMATION AND ANALYSIS TO AID IN THE
REDUCTION OF  MISHAPS, THIS IS THE FY04 FOURTH QUARTER REPORT ON
PMV/RODS MISHAP RATES AND TRENDS.  THE PURPOSE BEHIND THIS
REPORT IS TO IDENTIFY THE LEADING CAUSE OF DEATH AND INJURY
TO OUR MARINES, PROVIDE TOOLS AND RESOURCES TO ADDRESS THEM
AND MOST IMPORTANTLY, SAVE LIVES.

A.COMMARFORLANT NARRATIVES BY EVENT DATE:
 (1) 29 JUL 2004, 0605L,  18 YR OLD COMMARFORLANT MARINE (E3) DIED
IN MACCLESFIELD, NC DUE TO AN AUTOMOBILE CRASH. SNM WAS DRIVING WITH
A PASSENGER AT AN EXCESSIVE RATE OF SPEED WHEN HE LOST CONTROL OF
VEHICLE AND STRUCK
A CONCRETE BARRICADE. BOTH WERE EJECTED FROM THE VEHICLE AND IT
ROLLED ON TOP OF HIM KILLING HIM AT THE SCENE. UNKNOWN IF SEATBELTS
WERE IN USE OR IF ALCOHOL WAS INVOLVED.   
 (2) 1 AUG 2004, 0638L, ONE 21 YR OLD COMMARFORLANT MARINE (E3)
DIED IN PORTER TOWNSHIP, PA IN AN AUTOMOBILE CRASH. SNM WAS DRIVING
TO A FRIENDS HOUSE WHEN HIS VEHICLE VEERED OFF THE ROAD AND PLUNGED
INTO A SWOLLEN RIVER.
SNM'S BODY WAS LOCATED ON 4 AUG BY CIVILIAN AUTHORITIES FIVE MILES
FROM CRASH SITE. UNKNOWN IF DRIVER WAS WEARING SEATBELT. APPARENT
CAUSE OF DEATH WAS DROWNING. ROADS WERE WET AND RIVERS AND STREAMS
ENGORGED AS A RESULT OF HEAVY RAIN OVER PREVIOUS 48 HOURS. SPEED AND
FATIGUE WERE ALSO CONTRIBUTING FACTORS TO ACCIDENT. SNM ATTEMPTED TO
DRIVE ONE HUNDRED AND TWENTY MILES ON THREE AND HALF HOURS OF SLEEP.
 (3) 17 AUG 2004, 0539L, ONE 20 YR OLD COMMARFORLANT MARINE (E3)
DIED IN NEWBURG MD DUE TO AN AUTOMOBILE CRASH. SNM LOST CONTROL OF
HIS MAZDA RX7 ON POPES CREEK RD AND LEFT THE ROAD TO THE LEFT. THE
VEHICLE THEN HIT
EMBANKMENT CAUSING IT TO FLIP AND REENTER ROAD. VEHICLE SKIDDED ON
ROAD UPSIDE DOWN, FLIPPED COMPLETELY OVER ONCE MORE AND CAME TO REST
UPON HITTING EMBANKMENT ON RIGHT HAND SIDE OF ROAD. SNM WAS NOT
WEARING SEAT BELT AND WAS THROWN FROM POV.  SNM WAS PRONOUNCED DEAD
AT THE SCENE OF ACCIDENT.
 (4) 28 AUG 2004, 0250L, ONE 24 YR OLD COMMARFORLANT MARINE (E4)
DIED IN NORTH TOPSAIL, NC DUE TO BEING HIT BY A PMV. SNM WAS
RETURNING HOME AFTER SPENDING THE EVENING WITH A GROUP OF FRIENDS AT
A BAR ON HWY 210.  SNM WAS LEAVING THE
BAR WHEN HE ATTEMPTED TO CROSS THE STREET.  AN ONCOMING VEHICLE
STRUCK SNM.  EMS STATED THAT ALCOHOL WAS CONSUMED BY SNM, WHICH MAY
HAVE CAUSED LACK OF GOOD SITUATIONAL AWARENESS. SNM WAS PRONOUNCED
DEAD AT THE SCENE. 
 (5) 7 SEP 2004, 0230L, 2 COMMARFORLANT MARINES (ONE 22 YR OLD E3
AND ONE 20 YR OLD E3) & 1 CIV FEMALE DIED IN CRAWFORDVILLE, A IN AN
AUTOMOBILE CRASH. THEY WERE IN TORRENTIAL RAIN, THE DRIVER, LOST
CONTROL OF THE VEHICLE AND
HYDROPLANED INTO A TREE. THE DRIVER AND 2 PASSENGERS WERE KILLED
INSTANTLY.  ONE ADDITIONAL MARINE PASSENGER SUSTAINED MINOR BRUISES
AND WAS RELEASED FROM THE HOSPITAL. ST PARKER STATED THERE WAS NO
EVIDENCE INDICATING ALCOHOL WAS A FACTOR.
 (6) 16 SEP 2004, 0235L, ONE 27 YR OLD COMMARFORLANT MARINE (E6)
DIED IN RICHLANDS, NC DUE TO AN AUTOMOBILE CRASH. SNM WAS TRAVELING
IN RICHLANDS, NC AND LOST CONTROL OF HIS PRIVATELY OWNED VEHICLE.
ACCORDING TO THE HIGHWAY
PATROLMAN, HE DIED ON IMPACT. THE TROOPER BELIEVES THAT SNM WAS
TRAVELING AT AN EXCESSIVE SPEED AS HE WAS GOING AROUND A CURVE AND
HE WAS NOT WEARING A SEATBELT. THE TROOPER STATES THAT THERE WAS A
STRONG ODOR OF ALCOHOL PRESENT.
B.COMMARFORPAC NARRATIVES BY EVENT DATE:
 (1) 15 JUL 2004, 0130L, ONE 19 YR OLD COMMARFORPAC MARINE
(E2)DIED IN OCEANSIDE, CA DUE TO AN AUTOMOBILE ACCIDENT. SNM WAS
DRIVING HIS VEHICLE WITH TWO PASSENGERS (ONE E3 AND ONE CIVILIAN
FEMALE) AT AN EXCESSIVE RATE OF SPEED. AFTER LOSING CONTROL THE
VEHICLE WENT OFF THE ROAD AND ROLLED THROUGH A DITCH LANDING ON ITS
HOOD AND CAUGHT FIRE. THE SNM WAS TRAPPED INSIDE THE CAR BUT
EVENTUALLY WAS ABLE TO ESCAPE. SNM SUFFERED SECOND AND THIRD DEGREE
BURNS ON OVER 50 PERCENT OF HIS BODY AND WAS LATER PRONOUNCED DEAD.
THE MARINE PASSENGER WAS EJECTED FROM THE BACKSEAT AND SUFFERED
FRACTURED RIBS AND INJURIES TO THE LUMBAR REGIONS OF HIS BACK. THE
CIVILIAN PASSENGER HAD SUSTAINED BLUNT HEAD TRAUMA AND WAS ALSO
KILLED IN CRASH. IT IS UNKNOWN IF SEATBELTS WERE USED. WITNESSES
STATE THAT THE MARINES CONSUMED ALCOHOL IN THEIR BARRACKS BEFORE
LEAVING ON LIBERTY.
 (2) 16 AUG 2004, 1126L, ONE 22 YR OLD COMMARFORPAC MARINE (E4)
DIED IN MIRAMAR, CA DUE TO A MOTORCYCLE ACCIDENT.  SNM DROVE HIS GXR
600 SUZUKI MOTORCYCLE INTO THE BACK OF ANOTHER VEHICLE.  SNM FELL OF
THE MOTORCYCLE AND HIS HELMET CAME OFF CAUSING HIM TO SERIOUSLY
INJURED HIS HEAD.  SNM SUFFERED AN EPIDURAL AND SUBDURAL HEMATOMA
AND A BASILAR SKULL FRACTURE AND WAS LATER PRONOUNCED BRAIN DEAD. IF
SNM'S HELMET HAD NOT DISLODGED, HE WOULD HAVE WALKED AWAY WITH JUST
A FRACTURED THUMB AND ROAD RASH.
 (3) 29 AUG 2004, 0230L, ONE 19 YR OLD COMMARFORPAC MARINE (E3)
DIED IN PHOENIX, AZ IN AN AUTOMOBILE CRASH.  WHILE RETURNING TO 29
PALMS FROM LEAVE, TRAVELING I-10 WEST OUTSIDE OF PHOENIX, SNM WAS
SIDESWIPED BY A CIVILIAN VEHICLE. PMO REPORTS THE CIVILIAN MAY HAVE
BEEN UNDER THE INFLUENCE OF ALCOHOL AT THE TIME. THE ARIZONA STATE
POLICE AT THE ACCIDENT SITE STATED THE SNM WAS FATALLY INJURED
WHEN THE ROOF OF THE TRUCK HE WAS DRIVING COLLAPSED AFTER VEHICLE
ROLLED, CAUSING MASSIVE HEAD INJURIES. SNM WAS PARTIALLY EJECTED.
 (4) 1 SEP 2004, 1245L, ONE 23 YR OLD COMMARFORPAC MARINE (E4)
DIED IN CARLSBAD, CA DUE TO AN AUTOMOBILE CRASH. SNM WAS GOING ABOUT
100 MPH RACING ANOTHER VEHICLE WHEN HE LOST CONTROL OF HIS HONDA
CRX. SNM HIT A CONCRETE MIXER IN THE CENTER DIVIDER. SNM WAS
PRONOUNCED DEAD ON THE SCENE. HIS CIVILIAN PASSENGER WAS ALSO KILLED.
 (5) 16 SEP 2004, 0052L, ONE 19 YR OLD COMMARFORPAC MARINE (E2)
DIED IN OKINAWA, JAPAN DUE TO BEING HIT BY A PMV. A LOCAL NATIONAL
DRIVING NORTH ON THE OKINAWA EXPRESSWAY BETWEEN EXITS 8 AND 9 SAW
WHAT HE THOUGHT WAS A LOG IN THE ROAD, SWERVED HIS VEHICLE TO AVOID,
BUT HIT IT.  THE LOCAL NATIONAL THEN STOPPED AND BACKED UP, AND IT
TURNED OUT TO BE THE BODY OF THE SNM.  AT 0110L THE SNM WAS
TRANSPORTED TO HOKUBU HOSPITAL IN NAGO, AND PRONOUNCED DEAD.
C.MCCDC NARRATIVES BY EVENT DATE:
 (1) 8 JUL 2004, 0550L, ONE 27 YR OLD MCCDC MARINE (E6) DIED IN
ESCAMBIA, FL DUE TO A MOTORCYCLE ACCIDENT. SNM WAS ENROUTE TO WORK
TRAVELING SOUTH ON NEW WARRINGTON ROAD, PENSACOLA FL ON A PRIVATELY
OWNED MOTORCYCLE.  SNM IMPACTED A GUARD RAIL AT A HIGH RATE OF
SPEED AND WAS THROWN APPROX 100 FEET. SNM WAS PRONOUNCED DECEASED
UPON ARRIVAL OF EMERGENCY MED SERVICES.
D.MARCORMOBCOM NARRATIVES BY EVENT DATE:
 (1) 29 JUL 2004, 1630L, ONE 20 YR OLD MARCORMOBCOM MARINE (E3)
DIED IN IDAHO CITY, ID DUE TO A MOTORCYCLE ACCIDENT. SNM DROVE HIS
MOTORCYCLE LEFT OF CENTER, STRIKING A CHEVROLET TAHOE. ANOTHER
MOTORCYCLE, DRIVEN BY A FRIEND RIDING BEHIND SNM ON ANOTHER
MOTORCYCLE, THEN STRUCK SNM. SNM WAS DECLARED DEAD AT THE SCENE.
SNM'S FRIEND INJURED BOTH LEGS, BUT SURVIVED. BOTH SNM WERE WEARING
HELMETS AND FULL PROTECTIVE GEAR.

B. 7% OF MV FATALITIES INVOLVED AUTOMOBILE PASSENGERS.
C. 21% OF MV FATALITIES INVOLVED MOTORCYCLE OPERATORS.
D. 50% OF MV FATALITIES OCCURRED IN THE 18-20 YEAR-OLD AGE CATEGORY.
E. 36% OF MV FATALITIES OCCURRED IN THE 21-25 YEAR-OLD AGE CATEGORY.
F. 79% OF MV FATALITIES OCCURRED BETWEEN THE HOURS OF 0030-0629.
G. 57% OF MV FATALITIES INVOLVED LOSS OF VEHICLE CONTROL BY THE
OPERATOR.
H. 29% OF MV FATALITIES INVOLVED EXCESSIVE SPEED.
I. 29% OF MV FATALITIES INVOLVED MULTIPLE-VEHICLE COLLISIONS.
J. 50% OF MV FATALITIES INVOLVED SINGLE-VEHICLE COLLISIONS.
K. 21% OF MV FATALITIES INVOLVED ALCOHOL/DRUG USE.
L. 14% OF 4-WHEEL MV FATALITIES INVOLVED FAILURE TO WEAR SEATBELTS.
M. 21% OF 4-WHEEL MV FATALITIES INVOLVED EJECTION FROM THE VEHICLE.
