CAIRS Basic Reports

You can access the CAIRS Basic Reports page (shown below) from the CAIRS Database Tools page by selecting the Basic Reports hyperlink. Report options include:

• accident forms
• exposure reports
• property damage descriptions
• statistical reports

(Note that a Basic Reports hyperlink is available at the bottom of most pages in CAIRS. You can click on this hyperlink to return to the CAIRS Basic Reports page.)

5484.3 Accident Form Page

The 5484.3 Accident Form page is accessed from the CAIRS Basic Reports page by selecting the 5484.3 Form hyperlink.

The 5484.3 Accident Form page displays the form selection area (shown below). Within the selection area, users can choose the organization, accident date, case number, and detail that will be reflected in the forms generated by CAIRS.

• Organization is selected by entering the organization code in the Organization edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is all the organizations under the user's jurisdiction.

• Case Numbers are selected by entering the case number in the Case Number edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is all cases.

• Accident Date is selected by entering the date in the Date edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is the previous twleve months.

• Form detail is selected with the Detail selection box. The system default for this selection box is Full report.

REMEMBER THAT IF NO SELECTIONS ARE MADE, THE DEFAULTS WILL BE PROCESSED (i.e., all forms for the previous twelve months for all organizations under the user's jurisdiction).

Once selections are made, they are processed by clicking on either the or command button.

When the Count Cases command button is clicked, the system counts the number of cases selected. The page is recreated with the count displayed at the top of the selection area, as shown in the figure below.

When the Continue command button is clicked, a listing of 5484.3 forms that meet the criteria is generated and displayed on the screen (shown below).

The following information is displayed in the listing:

• Organization Code (Organization)
• Case Number (Case ID)
• Accident Date
• One-line Description: The description is hyperlinked to the individual case records. Note that system performance will be better if a new window is opened to view the individual case record.
• Date the report was last modified in CAIRS (Date of Last Revision)

The listing of individual forms can also be printed or saved for future use.

Exposure Reports

There are three types of reports that display the workhours, vehicle usage, and property valuation reported into CAIRS:

• Workhours and Vehicle Usage by Summary
• Workhours and Vehicle Usage by Organization
• Property Valuation

Workhours and Vehicle Usage by Summary

The Workhours and Vehicle Usage by Summary page is accessed from the CAIRS Basic Reports page by selecting the By Summary hyperlink.

The Workhours and Vehicle Usage by Summary page displays the report selection area (shown below). Within the selection area, users can choose the level of summary (Field Office, Operation Type, or All DOE) and calendar year that will be reflected in the report generated by CAIRS.

• Summary level is specified by selecting the desired radio button. If the Field Office radio button is selected, a field office code(s) can be entered into the edit box; however, if an entry is NOT made in the Summary Level edit box, the default is the field office associated with the user's jurisdiction. If the Operation Type radio button is selected, then an operation type code(s) can be entered into the Summary Level edit box, the default is all operation types. Operators can be used to further refine the criteria. The system default is All DOE.

• Calendar year is selected by entering the date in the Date edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is the current calendar year.

REMEMBER THAT IF NO SELECTIONS ARE MADE, THE DEFAULTS WILL BE PROCESSED (i.e., All DOE for the current calendar year).

Once selections are made, they are processed by clicking on the command button. When the command button is clicked, the exposure report is generated and displayed on the screen (shown below). For every year reflected in the report, both quarterly (Q1, Q2, Q3, Q4) and total year exposure information is displayed.

The report displays the following information:

• Total workhours
• Number of fixed-wing aircraft and hours of flight time
• Number of rotary-wing aircraft and hours of flight time
• Number of buses and miles traveled
• Number of government cars, light trucks, vans & motorcycles and miles traveled
• Number of marine craft and hours operated
• Number of rail conveyences and miles traveled
• Number of trucks (1 ton and over) and miles traveled

Reports can also be printed or saved for future use.

Note that the Q1, Q2, Q3, Q4, and Total column headings are hyperlinked to reports that display the same information at the reporting organization level of detail.

Workhours and Vehicle Usage by Organization

The Workhours and Vehicle Usage by Organization page is accessed from the CAIRS Basic Reports page by selecting the Or By Organization hyperlink.

The Workhours and Vehicle Usage by Organization page displays the report selection area (shown below). Within the selection area, users can choose the reporting organization and calendar year that will be reflected in the report generated by CAIRS.

• Organization is selected by entering the organization code in the Reporting Organization edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is all the organizations under the user's jurisdiction.

• Calendar year is selected by entering the date in the Date edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is the current calendar year.

REMEMBER THAT IF MAKE NO SELECTIONS, THE DEFAULTS WILL BE PROCESSED (i.e., All organizations under the user's jurisdiction for the current calendar year).

• ORG: Organization code
• Year: Calendar Year
• HOURS: Total workhours
• AF: Number of fixed-wing aircraft and hours of flight time
• AR: Number of rotary-wing aircraft and hours of flight time
• BU: Number of buses and miles traveled

The Property Valuation page displays the report selection area (shown below). Within the selection area, users can choose the organization level (Organization, Operations Type, or All DOE) and calendar year that will be reflected in the report generated by CAIRS.

• Organization level is specified by selecting the desired radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the edit box; however, if an entry is NOT made in the edit box, the default is all organizations under the user's jurisdiction. If the Operation Type radio button is selected then an operation type code(s) can be entered into the edit box; however, if an entry is NOT made in the edit box, the default is all operation types. Note that operators can be used to further refine the criteria.

• Calendar year is selected by entering the date in the Date edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is the current calendar year.

Property Damage Descriptions

The Property Damage Descriptions page is accessed from the CAIRS Basic Reports page by selecting the Property Damage Descriptions hyperlink. The report displays information pertaining to property damage reports showing DOE losses of $1,000 or more.

The Property Damage Descriptions page displays the report selection area (shown below). Within the selection area, users can choose the organization, operation type, year, and summary options that will be reflected in the report generated by CAIRS.

• Organization is selected by entering the organization code in the Organization edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is all organizations under the user's jurisdiction.

• Operation Type is selected with the Op Type selection box. The system default is all operation types.

• Year is selected by entering the date in the Year edit box. Operators can be used to further refine the criteria. If this edit box is left blank, the system default is all years. Specify calendar or fiscal year by selecting either the Calendar or Fiscal radio button. The system default is calendar year.

• This report is always grouped by field organization. However, users can specify levels of summary (Organization, Op Type, and P/V Code) under the field organization by selecting one or more Summary Options check boxes. The system default is organization.

REMEMBER THAT IF NO SELECTIONS ARE MADE, THE DEFAULTS WILL BE PROCESSED (i.e., All organizations under the user's jurisdiction for all calendar years and summarized by reporting organization).

Once selections are made, they are processed by clicking on the command button. When the command button is clicked, the exposure report is generated and displayed on the screen (shown below).

The report displays the following information:

• Org Code: Organization code
• Op Type: Operation Type
• P/V Code: Property Loss Type
• Accident Date
• Dollar Loss: DOE Dollar Loss
• Accident Description: The description is hyperlinked to the individual case records. Note that system performance will be better if a new window is opened to view the record.

Reports can also be printed or saved for future use.

Statistical Reports

There are four types of reports that display statistical information:

• Injury/Illness Experience
• Property Damage Experience
• Vehicle Experience
• Profiles

Injury/Illness Experience

The Injury/Illness Experience page is accessed from the CAIRS Basic Reports page by selecting the Injury/Illness hyperlink.

The Injury/Illness Experience page displays the report selection area (shown below). Within the selection area, users can choose the organization level (Organization, Operation Type, or All DOE), year, statistical data, and summary options that will be reflected in the report generated by CAIRS.

• Organization level is specified by selecting the desired radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the Organization Level edit box; however, if an entry is NOT made in the edit box, the default is all organizations under the user's jurisdiction. If the Operation Type radio button is selected then an operation type code(s) can be entered into the Organization Level edit box; however, if an entry is NOT made in the edit box, the default is all operation types. Note that operators can be used to further refine the criteria.

• Year and quarter is selected by entering the year in the Year edit box and quarter in the Quarter edit box. Operators can be used to further refine the criteria. If the edit boxes are left blank, the system defaults are current year and all quarters. In addition, calendar or fiscal year are specified by selecting either the Calendar or Fiscal radio button. The system default is calendar year.

• Statistical data for the report is selected with the Display These Columns check boxes and include the following selections:

  Hours

  Total Workhours

  FTE

  Full-time Equivalent Employees

  TRC

  Number of Total Recordable Cases

  TRC Rate

  Total Recordable Case Rate

  LWC

  Number of Lost Workday Cases

  LWC Rate

  Lost Workday Case Rate

  DAFWC

  Number of Days Away From Work Cases

  NFC

  Number of Nonfatal Cases Without Lost Workdays

  A weighting method that allows for comparisons between reporting units.

  Note that columns for Organization and Year-Qtr are always displayed in the report. Data displayed on the Year-Qtr lines (e.g., 1995-1) reflects experience for that quarter only. Year-to-date information is displayed on the summary (total) line.

  The system defaults are Hours, FTE, TRC, TRC Rate, LWC, LWC Rate, LWD, LWD Rate, and the Cost Index.

• One or more summary options can be specified in the Summary Options edit box using the number assigned to each option (i.e., 1 for operation type, 2 for organization, and 3 for year/quarter). Multiple entries are separated by commas. For example, to first summarize by operation type and then organization, enter 1,2 in the edit box. The system default is no summary options.

REMEMBER THAT IF NO SELECTIONS ARE MADE, THE DEFAULTS WILL BE PROCESSED (i.e., Hours, FTEs, TRCs, TRC Rate, LWCs, LWC Rate, LWDs, LWD Rate, and Cost Index for All DOE for the current calendar year).

Once selections are made, they are processed by clicking on the command button. When the command button is clicked, the statistical report is generated and displayed on the screen (shown below). The report will display the statistical information selected on the previous page.

Reports can also be printed or saved for future use.

Property Damage Experience

The Property Damage Experience page is accessed from the CAIRS Basic Reports page by selecting the Property hyperlink.

The Property Damage Experience page displays the report selection area (shown below). Within the selection area, users can choose the organization level (Organization, Operation Type, or All DOE), year, and summary options that will be reflected in the report generated by CAIRS.

• Organization level is specified by selecting the desired radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the Summary Level edit box. If this edit box is left blank, the system default is all the organizations under the user's jurisdiction. • Non-Fire Loss Rate
• Total property loss cases
• Total DOE loss
• Property Loss Rate
• Total property valuation (in $1,000s)

Vehicle Experience

The Vehicle Experience page is accessed from the CAIRS Basic Reports page by selecting the Vehicle hyperlink.

The Vehicle Experience page displays the report selection area (shown below). Within the selection area, users can choose the organization level (Organization, Operation Type, or All DOE), year, and summary options that will be reflected in the report generated by CAIRS.

• Organization level is specified by selecting the desired radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the Summary Level edit box. If this edit box is left blank, the system default is all the organizations under the user's jurisdiction. Note that columns for Organization and Year-Qtr are always displayed in the report. Data displayed on the Year-Qtr lines (e.g., 1995-1) reflects experience for that quarter only. Year-to-date information is displayed on the summary (total) line.

Profiles

The Profiles page is accessed from the CAIRS Basic Reports page by selecting the Profiles hyperlink.

The Profiles page displays the report selection area (shown below). Within the selection area, users can choose the profile type (injury/illness, property, or vehicle), criteria for the detailed information displayed on the report, and criteria for the summary information displayed on the report.

• Profile type is selected by clicking on the Injury/Illness, Property, or Vehicle radio buttons. Profile type determines the type of report that is generated by CAIRS (i.e., injury/illness, property, or vehicle). The system default is Injury/Illness.

• Criteria for the detailed information is selected from the Level of Detail area. The organization detail is selected by clicking on either the All DOE, Organization, or Operation Type radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the edit box to the right of the radio button; however, if an entry is NOT made in the edit box, the default is all DOE organizations. If the Operation Type radio button is selected then an operation type code(s) can be entered into the edit box at the right of the radio button; however, if an entry is NOT made in the edit box, the default is all operation types. Note that operators can be used to further refine the criteria.

  The year(s) to be detailed in the report are specified in the Year edit box. Operators can be used to further refine the criteria. If the edit box is left blank, the system default is the current year and previous five years. Calendar or fiscal year can be selected by clicking on either the Calendar or Fiscal radio button (calendar year is the default).

• Criteria for summary information is selected from the Comparison Level area. The summary organization is selected by clicking on either the ALL DOE, Organization, Field Office, or Operation Type radio button. The system default is All DOE. If the Organization radio button is selected, then an organization code(s) can be entered into the edit box to the right of the radio button; however, if an entry is NOT made in the edit box, the default is all organizations. If the Operation Type radio button is selected then an operation type code(s) can be entered into the edit box; however, if an entry is NOT made in the edit box, the default is all operation types. If the Field Office radio button is selected then a field office code(s) can be entered into the edit box; however, if an entry is NOT made in the edit box, the default is all field offices. Note that operators can be used to further refine the criteria.

  The years to be summarized in the report are specified in the Year edit box. Operators can be used to further refine the criteria. If the edit box is left blank, the system default is the previous five years. Calendar or fiscal year can be selected by clicking on either the Calendar or Fiscal radio button (calendar year is the default).

REMEMBER THAT IF NO SELECTIONS ARE MADE, THE DEFAULTS WILL BE PROCESSED (i.e., Injury profile and level of detail, All DOE for current year and previous five years, and comparison level, All DOE and previous five years).

Once selections are made, they are processed by clicking on the command button. When the command button is clicked, the injury/illness, property, or vehicle statistical report is generated and displayed on the screen. Reports can also be printed or saved for future use.

The injury/illness report (shown below) will display the following information:

• Year
• Full-time Equivalent Employees (FTEs)
• Total Workhours
• Number of Total Recordable Cases (TRC)
• Total Recordable Case Rate (TRC Rate)
• Number of Illness Cases (IC)
• Illness Case Rate (IC Rate
• Number of Lost Workday Cases (LWC)
• Lost Workday Case Rate (LWC Rate)
• Number of Days Away From Work (WDL)
• Number of Restricted Workdays (WDLR)
• Total Number of Lost Workdays (LWD)
• Lost Workday Rate (LWD Rate)
