How do I get an appointment?
Neurology consultations and evaluations are available on a referral basis only and by appointment only, to beneficiaries aged 16 and older. Call (719)457-2273 to schedule an appointment.
Botox injections, Lumbar puncture, Electroencephalogram (EEG), Evoked Potential (EP), Nerve Conduction Velocity (NCV), and Electromyographic (EMG) studies are available on a referral basis and by appointment only to beneficiaries aged 16 and older. How do I get my medication refilled/renewed?
Call the Neurology Clinic (719)333-5037 to request a refill at least one week before you will run out of medication. Provide your name, sponsor's last 4, medication being refilled, dosage, and what pharmacy you would like to pick the medication up at to the front desk.
How do I obtain copies of medical information?
You will need to fill out and sign a medical release of information form with the Neurology office. All documentation of visits to the Neurology clinic is forwarded to the requesting location.
