If you wish to report what you think might be unauthorized work in waters or wetlands, you may print out this form and fax or mail it to our office. Please provide as much information as you can, especially regarding the exact location of the work and specifically who is performing the work (please provide the name of landowner and/or contractor, if known). Maps are very helpful. You may submit this information anonymously, if you wish. (in PDF format)
