Telephone Call In Information Cards

These cards are provided to assist the GSA Internal Fleet motor vehicle operators
of the information required at the time of each fueling for the telephone
call in procedure.

The cards have been sized to be placed in the vehicle key fob, wallet, credit
card case, or in the vehicle, for the motor vehicle operator's convenience.

Suggested Options for Printing the Telephone Call-In Information Card

Option 1 - Avery Set Up for Labels # 8163  2" x 4"

Step 1 - Put Avery 8163  2" x 4" labels in printer and print the document.
Step 2 - Place labels to index cards or credit card size paper and cut to size.

Option 2 -Print Copy on Plain Paper (8 1/2" x 11")

Step 1 - Put regular paper or heavier stock in your printer and print the document.
Step 2 - Cut to size for use.
