EFT (Electronic Fund Transfer)

If you do business with the Mobile District Corps of Engineers, you need to print and fill out the Direct Deposit Form UFC-DISB-4 (17K)
Please read the Instructions for completing Form UFC-DISB-4 (7K).
This letter (5K) provides you with the correct mailing address for the form.
Find out who should use the Electronic Payment/Direct Deposit (8K) process.

• Form UFC-DISP-4 can be used by travelers and vendors
• Vendor and travel EFT is strictly voluntary
• The UFC will accept only original signatures (i.e., the forms must be mailed - no faxes will be accepted)
• The forms must be mailed to the following address:

             U.S. Army COE Financial Center
             ATTN: Mr. Lee Autry
             5720 Integrity Drive
             Millington, TN 38054-5005

   NOTE:  The envelope must say "DO NOT OPEN IN THE MAILROOM"
