Meal Benefit Forms

Provided below are links to meal benefit form prototypes and translated documents for English and 18 other languages. 

Note, we may not have all the translated documents for each language, such as the CACFP flier.  Also, information in the English and Spanish version of the documents is updated regularly, according to changes in the CACFP regulations, but the (17)  other languages were last translated in 2000 and may contain out-of-date information.*

 *We have updated the information in the “For All Households” section of the CACFP Provider Letter (Tier I or Provider’s Own Children) and CACFP Parent Letter (Tier II Day Care Homes and Child Care Centers) to reflect the modified requirements for the duration of applications for free or reduced price meals. Therefore, once properly approved for free or reduced price benefits, a household will remain eligible for those benefits for a period not to exceed 12 months.
