  3. Do you work or attend school?

  4. When you need childcare, how do you find out who is available?

  5. Who cares for the children who are not school age?

  6. Is this a licensed provider or a relative?

  7. Would you use a center on the Reservation?

  8. How many hours per week would you need childcare services?

  9. Would you use evening or non-standard hours, childcare services?

  10. Are there any childcare services that you need, that are not currently being offered?

  11. Do you know that Carlton County, St. Louis County and the Fond du Lac Reservation each has a Child Care Assistance Program?

  12. Have you applied for any of these programs?

  13. Are the Parent co-payment fees at a reasonable rate for your income?

  14. Does the Fond du Lac hourly reimbursement rate ($2.20 and $2.05), reflect the current market rate for this community?

  15. Would you be interested in becoming a certified or a licensed Daycare Provider?
