Verification Documentation

Military sponsors need to verify that the orders on file with their child's name on it are current for the school year 2007/2008. If orders on file expire before August 27, 2007, extension orders must be provided during the re-registration period.

Sponsors who provide a Central Billing Letter should ask their agencies to provide them with one relevant to the 2007/2008 school year. Also bring in a valid copy of the sponsor's current ID card.

Emergency Contact: someone other than the sponsor or spouse. This should be someone who can drive and has access to a vehicle. Please make sure this person is aware that they are on record as being your emergency contact.

Immunization requirements for registration:

Elementary Students:

• DPT series – minimum of 3 doses, with the third shot administered after the 4 th birthday.
• Hepatitis B – 3 doses
• MMR – 2 doses, the second administered after the 4 th birthday.
• OPV (IPV) – 3 doses minimum, with 1 dose administered after the 4 th birthday.
• Varicella – History of disease or 1 dose of vaccine.
• Hepatitis A – 2 doses, or records showing the student having begun the series.

Middle School Students:

• All the above with the following additions :
• Tdap – after the 11 th birthday
• Menactra (MGC or MVC4) – after the 11 th birthday
